Concerned

First, I have tested positive for genital herpes but to the best of my knowledge I have never had a breakout or it was not noticeable.  About a week ago I noticed a small hard bump below the glans on the shaft of my penis.  The bump was not noticeable when my penis was flaccid. Today when I had an erection the bump was still there and seemed to have gotten bigger or widened.  It also is noticeable when I am flaccid. I do not have an open sore at the site of the bump and the bump seems to be under the skin.

As I said I have not knowingly experienced a herpes outbreak in the past. Could this be the beginning of an outbreak? Another STI? I tested negative in for other STIs when I was tested for herpes.  I have always worn a condom during sex so I am not sure if this is an STI or growth that I should be concerned about.  Your comments and advice would be appreciated.

Went to my internist today. He examined me and said he not see anything and that it was probably a blood vessel.

Welcome back to our Forum.  I reviewed your prior exchange with Dr. Handsfield.  

Given your use of condoms other than for receipt of unprotected oral sex, the likelihood that this is a bacterial STD is miniscule and not a concern.  That the lesion was noted a week ago and is now more apparent suggests that is should be evaluated.  There are rare instances in which HPV (genital warts) get be transmitted through oral sex but this is quite uncommon.  The lesion could be a non-STD skin process like folliculitis or a sebaceous cyst.  Finally, there is a possibility this could be the HSV infection that you became aware of through blood testing (about half of people who have positive blood tests then note recurrences which had been previously overlooked in the 6 months following their diagnosis).  

My advice is to have your health care provider take a look.  From your description I am betting this is a cyst or other non-infectious process but there is no substitute for evaluation by a trained provider.  If this was a HSV recurrence it would be unusual for it to have lasted a week and as I said, it would be quite unusual to have gotten a wart this way.  If there is any question about HSV, your provider can take a culture or PCR test from the area even if there is not a blister or ulcer.
