Confused What Could've Caused My Symptoms

1 week ago I had protected oral sex from an escort. There was foreplay where she groped my penis with her hand before putting on a condom and giving me rough oral sex which ended with me ejaculating in her mouth with the condom on. Later on in that day I had achiness in the left testicle and a sensitive, cold, irritated sensation at the very upper left part of my penis and head. This persisted in intensity for two-three days in which the left side of the urethra at the penis head irritated when initially urinating. Currently, the irritation and sensitivity of the upper left part of my penis have decreased but I still have achiness in my left testicle or gland that increases and decreases in severity throughout the day. I still have some irritation or slight rough feeling in the same area in the urethra when initially urinating although much better now. I sometimes get a pre-*** like clear discharge in the morning (small amount), and a feeling like I have discharge throughout the day even though when I check I see nothing but could be because it has dried on my underwear. I’m confused about my symptoms since from everything I’ve read on these forums there’s no possible way to catch a std from the exposure I described. So my questions are:

1. If the condom broke, I or the escort would have definitely known it correct?

2. If a condom is put on incorrectly meaning inside-out, would it still be effective?

3. Based on my exposure and description can I safely rule out gonorrhea and chlamydia?

4.If I want to test for NGU, how long would I have to wait from date of exposure and how do you test for it? Should I test for it?

5. If you do not think it's any STD, then can you guess as to what is causing my symptoms? Could it have been the mixture of semen, lubricant and bacteria on penis skin going into urethra causing these symptoms? Or something else?

6. Should I ask my doctor for antibiotics or any other tests?

I continue to believe your symptoms are not due to any STD.  My comment about another possible cause was little more than a logic-based guess.  If you do have an STD, it originated from some sexual event other than the one described here.

I may sometimes have the clear pre-*** discharge during the day but definitely not an abnormal mucus or pus in which case I'm guessing that's ok. And I can safely rule out any STD from prior encounters since I've always used protection for everything and I was negative on all my last STD tests which were after my last sexual encounter. However, the consistent aching left testicle or left testicle gland, and the slight stinging, irritation in the upper left urethra area when urinating is somewhat disconcerting in combination with the timing of the onset of my symptoms. But it's reassuring reading your response, and if this persists since I have Kaiser insurance I can just contact my doctor.

And you said, "Most likely your penile discomfort is due to minor mechanical irritation from vigorous rubbing etc." So you were referring to the rubbing from the oral sex as the cause of my symptoms correct? And do you feel this is something that just will resolve over time if left alone?

Welcome to the STD forum.

You are exactly right when you say "everything I’ve read on these forums there’s no possible way to catch a std from the exposure I described".  Having had only condom protected oral sex plus hand-genital contact, you can be certain you acquired no STD during the event.  Further, your symptoms don't really suggest any STD.  Most likely your penile discomfort is due to minor mechanical irritation from vigorous rubbing etc.  Virtually all men experience "clear discharge in the morning" from time to time; it is pre-ejaculate fluid that accompanies normal nocturnal erections.

The only symptom that comes close is your "feeling" of discharge, but it seems obvious that nothing is there.  If there are actual stains you can see on your underwear, then perhaps I'm wrong about this -- but in that case, you should be able to see the abnormal mucus or pus in the penile opening at least some of the time.  Of course you should visit a health care provider if you remain convinced that you really are having penile discharge.  If you get examined, consider your local health department STD clinic as the most reliable source of expert care in most communities (at least in North America).  Go early in the day and avoid urinating that morning, for the most accurate exam for possible discharge.

I think that effectively answers all your questions, but to be sure:  1) A broken condom is impossible to miss.  2) Condoms are reversible.  How could it make a difference?  3,4) It is not possible you caught any of these 3 STDs during this event.  I recommend against testing for NGU or anything else.  5) See above.  6) Don't ask for treatment or any specific tests.  If you seek care, just describe the exposure, your symptoms, and your concerns -- then follow his or her advice.  But based on all you say here, you can expect nothing to be found.  (And if indeed you have an STD, you'll have to go back to other sexual encounters to explain it.)
