Constant worrying

Hi all,

Looking for some thoughts related to a situation I currently find myself in.

It all started a few months ago when I had protected vaginal and unprotected oral with a high risk individual. Within 5 days I developed conjunctivitis and that's when my woes started.
I had a test for Chlamydia, Gonorrhea, HIV and Syphillis 9 days after exposure - all negative.
I re-tested 4 weeks after exposure - all negative.
The conjunctivitis was still ongoing though and therefore anxiety started to build.

With various searching on the net I read about other symptoms and approximately 5 weeks after exposure I became concerned about a pain in my left testicle, and general groin area and felt like I needed to pee more frequently. Back for more of the above tests - all negative. With a wave of relief I decided to put this behind me.

A week later (6-7 weeks after exposure) I had unprotected vaginal sex with my partner and developed balanitis within 24 hours. Anxiety levels went through the roof. Went back to the clinic after 2 weeks (8-9 weeks after exposure) and repeated the usual urine and blood tests - all negative.

Decided to book an appointment with the clinic the following week as this was now an all consuming worry as conjunctivitis, testicle / groin discomfort and balanitis all still present. Having reviewed my history they determined a further std test was of no use and instead performed urine analysis and a swab of the eye. Results would then be available in 1 week.

In the interim I stupidly decided I definitely had chlamydia and took a 1g dose of azithromycin.

Results came back all negative. Relief and decided to put the matter to bed and move on.

A week later had unprotected sex with my partner again and balanitis recurred within 24 hours. Went to my doctor about the now returned testicle pain and balanitis. The doctor diagnosed a couple of cysts and thick epididymis.

I was referred to a urologist who reviewed me and said he thought I had a cyst and varicocele but wasnt concerned about epididymitis and scheduled me for an ultrasound, this was approximately 2 weeks after taking the antibiotics.

2 days later the feeling of needing to pee reached its maximum following a salt bath and applying an antifungal cream to the penis. Now 3 days on all symptoms appear to have disappeared with the exception of minor conjunctivitis and balanitis.

Any ideas as to what could have caused all these symptoms within the last 12 weeks?

Test results just came back as negative for MG therefore time to put the STD concerns to one side and focus on getting over this with a urologist.

I went to a private clinic today and provided a sample for MG testing. Should hopefully get the results on Friday and assuming all is well put this std concern behind me.

I want to start by saying I wish you and your partner well.

There's a good chance that what's going on is not the result of infection. You have mentioned that yourself above. Anxiety, stress, and guilt can be a lot to a body (and a mind, and therefore the body). With luck, that's what's going on for you, and with more luck, you can move past it.

However, if you read various things on the web and various postings here, it's pretty obvious that there's still a lot about NGU in general and about MG in particular that medical science still doesn't understand. That's unfortunate, because we all want answers, but it's to be understood, because it's a relatively newly discovered pathogen and human knowledge is limited--sadly.

It seems like you're in a spot where you'd really benefit from having a specific NAAT test for mycoplasma genitalium. I gather you're in England and it sounds like it's difficult for you to get one of these. If I were you, I'd double-down hard trying to find out how you can get one of these tests. I know others here on MedHelp from England have gotten one from the GUM, and I also know that in the US you can get one online and/or from many doctors--maybe you can do the same.

I have found the issue of MG confusing, but it DOES sound like you can have it without having a high enough WBC in your urethral swab to be considered to have NGU. If I'm wrong about that, I encourage anyone here on this forum to correct me. And it sounds like it can create some of the symptoms you're having (pain in groin/testicle comes up A LOT for people who think they have MG).

Again, I wish you the best. You know it may be anxiety and I hope that' s it, and that you can pass through it. But no doubt taking the MG test and getting a negative result would help you.

I've got discharge, it's just that I wasn't diagnosed as nsu I'm assuming due to low pmnl count.
So as of now I've got red eye (mild following keratoconjunctivitis developed 5 days after exposure), balanitis (6 weeks post exposure) and discharge (noticed 11 weeks post exposure).
So I'm wondering what if anything could cause all 3 symptoms to occur at the same time?

MG is not contained in the mouth for NGU, and NGU (NSU) will give discharge symptoms.

The std results for gon and chlamydia came back negative.
I saw a urolgist who suggested I take a 2 week course of cipro 500mg tice a day and he is also going to refer me to a rheumatologist in case my conjunctivitis, balanitis and urethritis are linked and therefore a sign of reactive arthritis. I've got an appointment booked for next week.

In the meantime I've got a question that I'm hoping someone can answer with regards to NSU diagnosis. My concern is that mycoplasma gentialium can be missed if you have a PMNL count of less than 5 and therefore NSU isnt diagnosed based on discharge alone, the std tests for gon and chlamydia then come back negative and therefore you are never tested for MG and furthermore you are never treated for NSU as it was discounted. Is this feasible?

Ok so the urologist confirmed no sign of epididymitis and essentially said all ok based on the ultra sound. However a few days later I noticed a discharge from the urethra. I made an appointment at the gum clinic and the discharge appeared to be getting more noticeable.
Had a swab and urine sample taken. The sample showed no pus / white blood cells and urine sample didn't appear to show any inflammation so awaiting further analysis for std's (only gon and chlamydia) so I am expecting these to be clear.
As the initial review under the microscope showed little or no white blood cells i wasn't prescribed any antibiotics and was advised the discharge is probably related to harmless oral bacteria that's got into the urethra.
Assuming the std tests come back negative I'm not sure what my next steps are?

Thanks that's reassuring and explains why the urologist wasn't overly concerned about epididymitis immediately and opted for the ultrasound.

Thanks. Thats what im constantly trying to tell myself but just as I think I've got over it, wham something else crops up.

How accurate are NAAT urine tests for STI's? Im assuming there is little chance that something was missed in the 4 tests since the exposure?

Not really, but can tell you it has nothing to do with a STD. Probably just anxiety/guilt.
