Difficult recovery

2 months ago I started having genital pain all around my boxer short area and leg pain and shocking pain to my feet with headaches.  I tested for STDs and it was all negative.  I feared my gf cheated.  My doctor said prostatitis and gave me cipro.  4 weeks later I wake up with discharge from my penis.  I go to the dr and get swabbed.  Comes back positive for Chlymadia.  Gives me dioxochicline for a week.  I still had discharge so he did another week.  I felt better but not normal.  He retested me for all STDs.  Still positive for chylamida. So I got a Z pack.  And a week passed and I still have pain in my urethra and testicles.  Burning feeling all over my genitals.  Got another Z pack yesterday and a shoot of rocephin.  I'm still in pain today.  Dr says he has no idea why I'm not cured.  I had my girl go 3 weeks ago and test and she was negative for all STDs except Chlymadia.  The clear drainage from my penis continues today and the tip of my penis burns and my anal area burns now.  My stomach is destroyed from antibiotics!! I could throw up.  Could herpes cause clear drainage?  What else might?  Why wouldn't this Chlymadia die with all that tx?I have slept with my gf since she was treated with Z pack 3 weeks ago. She has no symptoms.  She admitted cheating last fall but how can that be true when she just now gave me this?!  If she's telling the truth? Her tests are clear if she hasnt cheated in 10 months, but if not then she could have herpes, etc since this all falls in the 8-10 week window for results from my return could be inaccurate?  I'm reading what can cause this on l the CDC and it seems like it has to be something like herpes and chylamdia.  theres no way she had chylamida all these months and it just start now?! Or could Chlymadia alone do all this and again why aren't the meds working?  The pain in my urethra is all over but mostly close to the tip.  Especially on the base side.  It stings and tingles in the same spot now for a month.

Dr my test results. (I was taking famvir 2 weeks straight before my WB.  I had been taking it just occasionally before then.  I noticed on my WB from 2008 when I got type 1 that it was drawn at 16 weeks and I had been on Famvir daily at that time.)

I have had no classic sores.  However after sex yesterday I noticed a watery drainage on a red area on the rim of my penis (top side).  Now this is the exact area that felt like it was on fire in early July that started all this for me.  It had a very tiny drainage a few weeks back when I was very stressed as well, but quickly went away.  In this latest situation I wiped it dry and 30 sec later it was wet again.  No noticeable sore but my penis was still somewhat erect.  I dried it again got dressed and went to the dr.  By then my penis had gone down and no fluid was there.  
I have spasms in my inner thigh muscles that come and go as I mentioned before and these past few days its been very noticeable.  No pain.  Just jerking muscles.  Dr says it sounds like a neuro issue.  I saw PT Friday and they looked at my L5/S1 herniation and tested me.  Said it could possibly contribute to my symptoms. Not positive though.  Urologist last week says he thinks I have pelvic pain syndrome.  Says my prostate (which was his original diagnosis) should have been repaired from antibiotics.  So many opinions.

Maybe this is all type 1 genital?  In 2008 when I was first infected with type 1 there was a itchy bump on my pubic hair region that itched to high heaven.  It lasted 6 days and started to go away. (back then I wasn't such a worrier). Then I started having urethral pain and shocking pains.  I decided to see my dr.  He wasn't sure so he ran all the tests.  Only positive was my herpes IgM:  5.0, so the dr said herpes.  He then
ran several IgG tests over the weeks and they were negative until week 6 when I hit 1.15 on type 1.  Further testing had me below .90 for type 1 so at 16 weeks I did the WB to make sure and it was positive type 1.  I didn't test IgG positive for type 1 for over a year!  Now it's in the 4.0 range. Always type 2 negative.  Terri and others didn't think this was type 1 genital but maybe it was?  I have had symptoms over the years but never like this.  

Well thanks Dr for your time.  The results say I'm fine.  I started counseling to help me coup with the pain.

I'm sorry you are "so lost", but you shouldn't be. I've given you my best guess as to what's going on and my advice about how to resolve it.  We don't expect much from forum users, but I do expect you to read my replies and learn from them.  Otherwise why are you here?  

This thread is over until and unless you report the confirmatory test results I have suggested.  Any comments before then will be deleted without reply.

She just told me she was with the same guy in June.  So I was with her just weeks after she got infected I guess.  So she's negative at 14 weeks and I'm positive at 12.  I'm so lost.

Still, the evidence remains against her being infected and I remain skeptical about your test result.  But the WB will tell.  And of course both of you should be on the lookout for genuine genital herpes symptoms (blisters, sores, etc) and promptly get professionally evaluated, with PCR testing for HSV, if anything shows up.

In the meantime, be diplomatic and kind to your partner and sensitive to her feelings.  Most likely she's bummed out as it is, and you truly have no solid evidence she transmitted HSV-2 to you.  At this point I recommend flowers over recriminations.

I'll be happy to comment one last time if and when you care to return to report your WB result or the result of viroloigc testing in the event of symptoms.

Well her story just changed and she was with another guy in June.  Same guy.  So 12 weeks.

OK, it's time to calm down a bit.  These test results really don't make much sense, and as already noted (more than once), I remain confident that HSV is not the cause of the symptoms you have described.  My bet is that your current HSV-2 result is false, despite the strongly positive numerical result.  What you need now is an HSV Western blot test -- which, as you probably know, is the ultimate gold standard in HSV testing.  While you're at it, your girlfriend could have a WB test as well.  If WB testing confirms you have HSV-2, we can deal with the implications then.  In the meantime, I encourage you to stay mellow.  I'm betting the WB will show that neither you nor your partner has HSV-2 (assuming, of course, that your and your partner's sexual histories are accurate).

And what was your HSV-1 result?  Your 8 week test results, as posted on the herpes forum a few weeks ago, was HSV-1 4.28, HSV02 0.10.  My first concern is whether you (or the lab) have mixed up your current HSV-1 versus HSV-2 results.

Almost certainly your girlfriend does not have HSV-2.  If you in fact are infected, you're going to need to look to a different potential source and should not blame her.

My gf cheated in April.  I've been struggling with pains since early July.  We both tested 2 days ago.  Which puts her at 5 months.

Herpes does not explain any of the symptomsyou have described.  You have asymptomatic HSV-2 plus one or more other explanations for your symptoms.

If you would like to provide the details of your HSV-2 test (the specific test done and the numerical result) I may have other comments about it.

I doubt the "burning sensation" in your back or testicular "heat pain" are related to any of this, except perhaps as manifestations of anxiety over all this -- and the same may be true of your penile pain and most other symptoms.  I don't know what to make of clear discharge from the anus.  In any case, I'm sure that no STD explains any of this, and I'm also very confident you have nothing serious -- nothing that will ever harm you or a future sex partner.  The best course probably is to continue or resume your life as normally as possible, including sexual activity.  Avoid further antibiotic treatment unless and until a definite infection is documented -- which I doubt is the case.  Of course if your symptoms persist or you otherwise remain concerned, continue to work with your doctor.

Since it's clear you have no STD, which is the only topic for this forum, that will end this thread.  Best of luck.

Dr I appreciate the detailed response. For days now I've had a burning sensation in my lower back, burning feeling in my anal area and in my anus with clear discharge, pain in my penis especially in my urethra with some clear drainage, and general heat pain in my testicles.  Is this in line with what you are telling me?  Seems to be a constant issue now for 2 months or more.  Depressing!

Welcome to the forum.

I scanned your many discussions with Terri on the herpes forum and the various community forums.  This is a complex situation, but I think you have had three separate problems:  1) nonspecific (i.e. non-infectious) prostatitis that started 2-3 months ago; 2) chlamydial urethritis, acquired more recently and which has cleared up; and 3) antibiotic side effects, which may include an anal area yeast infection.

Your initial symptom of genital area pain probably was due to prostatitis, as your doctor thought.  When you developed non-clear urethral discharge more recently, it was due to the chlamydial infection, which you acquired after the other symptoms had been going on for quite a while.

Your positive repeat test for chlamydia doesn't mean anything and did not require additional antibiotics. The test detects chlamydial DNA, which persists for a while after the chlamydia are killed.  For that reason, a follow-up test for chlamydia should never be done sooner than 3 weeks after treatment.  The azithromycin and ceftriaxone (Rocephin) were superfluous.

Now that the chlamydia has been cured, you are continuing to have prostatitis symptoms, i.e. the clear urethral discharge.  Prostatitis isn't an STD.  Some cases of prostatitis are infectious, but many are not.  Antibotics like ciprofloxacin usually are tried, but often they make no difference -- as in your case.

I agree with Terri Warren on the herpes forum; herpes is not causing any of this. The symptoms are wrong and your HSV blood test results also are against it.  As for your anal symptoms, my guess is that you may have a yeast infection, a common consequence of antibiotic treatment, especially multiple courses of varied antibiotics as you have received.

As for the timing of your chlamydial infection, there's no mystery.  STDs are not transmitted with 100% efficiency -- i.e. it's often many exposures before the infection is transmitted; it is common for one member of a couple to have chlamydia for several weeks or even a few months before the partner catches it.  Also, ciprofloxacin protected you from chlamydia during the time you were taking that drug.  Or you may have been infected previously, without symptoms; the cipro cured it; but you then were infected again once you stopped that drug.  Second chlamydial infections often cause more prominent symptoms than the first infection.

My advice at this time is that you cool your jets a bit and stop trying to treat anything.  Non-infectious prostatitis isn't at all harmful -- it's a comfort issue without known long term health impact.  Left alone, the symptoms usually clear up over time.  It also will take time for any symptoms related to antibiotic over-use to clear up, such as anal irritation and your "stomach torn up" symptoms (whatever they are, you don't say exactly).  It may be worth trying an anti-fungal cream for the anal irritation; talk to your doctor about it, or try one of the over-the-counter treatments, e.g. Monistat or a similar product (look in the women's health section of the pharmacy).

As noted above, this is a complex situation.  The bottom line is that there is nothing dangerous going on here, and for sure you don't have herpes -- which until now appears to have been one of your main concerns.
