Does this sound like herpes?! Very worried

Ok guys...I am very nervous and I am seeing the doctor on monday.  This is the problem:

I have had a steady girlfriend for about 10 months, but we broke up quite recently.  Throughout our relationship, there were times my penis and genital area itched so badly that I was unable to sleep at night.  The skin on my penis and testicles has become much rougher and saggy. The skin on both the foreskin and the tip of the penis are wrinkled.  My other genital area skin is also very wrinkled and saggy.  The itch has pretty much stopped now and dry skin is peeling off.  Also, there were some very minute bumps on the area where my upper and lower lips connect.  My skin is slightly rougher around my mouth.  There is some itch around my mouth.  There is no pain except when I open my mouth really wide and there is 1 very small "sore."  It is white and it is probably because 1 of the small bumps was torn apart.  My lips are more wrinkled and feel numb.  This may be the 3rd recurrence.  Previous times I have always dismissed it as poison ivy.  I realize how stupid I was now.

Can anyone please help me get an idea of what this is before I see the doctor on Monday?  Thanks alot I truly appreciate it.

Neither your genital nor oral symptoms sound like herpes; more like an allergic reaction or other dermatitis of the genitals.  Yeast infection also is a possibility.

Follow through with your medical appointment, but don't lose too much sleep about herpes unless your doctor thinks otherwise when examining you.

Good luck--   HHH, MD
