Actually, herpes is much LESS easily transmitted than most STDs.  HPV probably is more easily transmitted, but both are less transmissible than gonorrhea or chlamydia.  Simple contact of the scrotum rarely transmits either one; in fact, herpes and warts are rarely seen on the scrotum.  Transmission of both infections probably involves "massaging" the virus into the skin.  Thus both herpes and warts usually occur in genital areas that get the most vigorous rubbing during sex:  head or shaft of penis in men, labia and vaginal opening in women.  It is impossible to give exact risks on transmission for a single episode of "normal sex"--the odds vary widely depending on numerous things, such as how active the infection is in the infected person (i.e., the amount of virus present, symptoms, etc).

I think not many people would find it very appealing to wear both underwear and a condom during sex.  The female condom, which spreads over the labia minor, probably prevents more skin-to-skin contact than male condoms do, so logically might be more effective in preventing exposure to HPV or HSV.  But no research has been done to know for sure.  And the female condom has not proved very popular, although some women and couples use it regularly.

No STD agent, including HPV, can pass through an intact condom.  Finally, yes, the immune system usually eradicates HPV (in over 90% of infected persons) within 6-12 months.
