Doc, I did something stupid.  I got drunk, went on craigslist, hooked up with a crossdresser (believe me, I know.  I'm a bi male but again I was intoxicated), and got an unprotected blowjob.  I am a 27 YO WM. It wasn't any longer than five minutes, and I pulled out and ejaculated outside the mouth.   I did not see any cold sores on him, he gave me oral on both my penis and testicles, but I am freaking out that I may have caught something.  HSV1 is the most prevalent on my mind, since it's the only one that stays with you and is incurable. I don't have any symptoms, but it's only been 10 hour since the fact.  Except for a slight burning sensation and I notice every little itch and move down there, I think it's mental. I keep checking it constantly.  I had an HIV scare back in 2006 after protected anal/oral so I know about STD's.  Again, just wondering what the odds for oral to genital HSV1 transmission are with no symptoms, what initial symptoms might be, how long I have to wait to see, testing, and if there is any chance at all of an HIV infection (I know it's a long shot but I want to make sure).  Thanks for any help.

Well, before we address your questions about oral sex, let me recommend that you stop checking yourself.  You will only "find" normal things and mistake them for STDs.  Now, on to your questions.

For starters, oral sex is an inefficient way to transmit STDs.  Of the bacterial STDs only gonorrhea and nongonococal urethritis (NGU) are transmitted through oral sex; chlamydia is not and without an obvious sore or lesion on your partner’s mouth, the chances of syphilis and herpes is likewise tiny.   If you had gotten gonorrhea or NGU you would have most likely developed symptoms of urethritis (penile infection).  Even if your partner had an STD (any STD and it is likely he did not), most exposures do not lead to infection.  In your case, your exposure was brief; but the tingling you mention could be a sign of NGU.  This can be addressed with a urethral swab or urine test (either one should be taken after having not voided for at least an hour, preferably 2) from a knowledgeable health care provider.  

Regarding herpes.  Indeed genital herpes due to HSV-1 is occasionally spread through oral sex but when it does, the person giving the sex usually has a cold sore and the person who gets infection then develops the sores/lesions of herpes 4-6 days after the exposure.  If no symptoms in that time, no risk.

Doc, by the way, after the oral I washed my penis and scrotum with hot water, urinated, bought two bottles of Listerine and poured it down there to be sure (I am pretty sure it doesn't work but it makes me feel better) and then came home and poured Hydrogen Peroxide down there and showered.  I know, I know...
