Dear Dr. H:

I have read through the STD General postings to find my risk assessment, but I still need some help.   Basically, my story is similar to others.  

I went to a massage parlor.   The two following things occured:

1.  To my surprise, the masseuse perform fellatio on me WITHOUT a condom.    The entire fellatio lasted NO more than 2 minutes.  
   Her mouth was NOT bleeding.  

   She did NOT have any cold sores, and actually her dental work (i.e., her teeth) was quite immaculate.


2.  I fingered the outside of her vagina for about 30-45 seconds. My finger did NOT enter her vagina, just only the outside.  She was not very moist at all, in fact, sort of dry.   I had a VERY, VERY, VERY, VERY tiny "knick" on my finger.  But it was NOT a cut, NOT even a paper cut.   Also, my finger was not bleeding.  Also, her vagina was NOT bleeding.

Okay, I'm trying to not be emotional about this, and this is my own risk assessment as follows.  HOWEVER, I need your help to simply confirm my own risk assessment, as I do NOT want to ignore this situation if I need to take action.

1.  Fellatio.   My risk assessment is as follows.  

     A.  The risk for STDs such as gonnorhea and chlymydia is extremely low, but not impossible.   Either way, they are curable.    I can take a urine test for this for peace of mind.

     B.  The risk for Herpes is theorectical, and I can simply get a 3-month Herpes test for my peace of mind.

     C.  The risk of HIV is zero, because the only fluid that I came in contact with is saliva.   This is so, even though I am not circumcized.  Thus, no need to take an HIV test for this 120 second fellatio that she performed on me.   PLEASE COMMENT ON THIS ONE AS THIS ONE WORRIES ME THE MOST.

2.  Fingering.  

    HIV does not go through intact skin.   Fingering is only risky as to HIV if the person doing the fingering has an "open and fresh" wound.  Even fingering with a small paper cut is really not risky.  

    Having a VERY, VERY, VERY minor "knick" on my finger that is NOT open, that is not bleeding is not an HIV-risk.

    Thus, I do not have an HIV risk here.


Doctor, please tell me if I am wrong on any of these.  I am trying to be reasonable about this; HOWEVER, if any of these two incident has an HIV risk, then I need to know.  I definitely do not want to be non-challant, especially since I have unprotected sex with my fiancee (this is the ONLY time that I have strayed.  For example, if I need a PEP or something like that, please tell me.

To be 100% sure as possible, do you recommend that I go to my Health Department and get begin PEP?

I'm almost at 48 hours after this experience.

Hi,

Recovering SA here too, though I like not to call myself an addict. I prefer the moniker "reformed tramp." I'm at 5 months without a slip, though this has simply driven my partner crazy because I need to have sex with her 3 times a day or I lose my mind.

Anyway, it can be done -- hang in there! I didn't have a habit of hiring prostitutes but I did have a problem with picking up new sex partners constantly; it took me a whole year to get down to one partner. It can be done!

The Dr. will probably need you to pay the $10 and start a new thread.  I am in "SA" recovery as well and have slipped a couple of times.

The deal is unprotected oral doesn't carry any HIV risk, but look for sores or dischard in the next few days.  There is a tiny, very small, risk ok HSV1 or gonerrea.  But those are risks are very low and if you have no discharge or sore in the next 10 -14 days your OK.  

But get back on track with that recovery.  I know it is difficult, but I also know it can be done.  Stay strong.

Dear Dr H.,

First off I would like to commend and thank you for performing the public service that you are here.  I am a struggling sex addict and alcoholic.  Been to a rehab in Arizona specializing in my problems, have managed to stay sober from alcohol to date, but sex remains a struggle.  I am attending "S" related 12 step meetings and working a program.  

Recently however I had a major slip.  I received unprotected fellatio from a prostitute who (I come to find out now) is purported to be HIV+.  Once again I have to sweat the 30 days for my initial HIV screen, but I was hoping you could comment now.

I think we established that there is NO risk, because there was no blood involved.  And

I know you want the dr. to answer. Mabyber he will.  But the research I have done and what I have heard the Dr. say is there is no risk to the insertive partner in oral.  There is some risk to the receptive partner especially if ejaculation occurs, but if ANY one has gotten HIV form heterosexual insertive oral (or MSM oral for that matter) it might have happned just a handful of times in the whole 20+ years of the epedemic.  And think of all the unprotected blow jobs that happen every day in this country, tens of thousnads no doubt and do not have ANY cases of HIV passed to the insertive partner should be enough for you to move on.

No, I did not see any open sores in her mouth.  Nor did I see blood on her mouth or on my penis.  

I was merely being hypothetical.  I guess there is a danger in being hypothetical, because anything can happen in the hypothethical world, even HIV transmission via kissing (for example).

Okay, maybe I just found the edge of my comfort zone.  

Sounds like the answer continues to be NO RISK in this specific incident.

Please look at my posting:  I said she did not have blood in her mouth.  The question is "what about open sores," and the HIV attaching itself to open sore in the mouth?

You get HIV from blood, vaginal secretions and semen entering into your blood stream either directly or through mucous membranes  .  So unless the open sores where bleeding, no you can't it from that.  And not only that, it would take more than a drop or two of blood to find it's way somehow into your ureatha.  Did you see a sore in her mouth or are you being hypothetical since you didnt mention it in your first post?  Like I said, it is possible that her sore had blood on it and it got in your uretha and you have HIV.  No scientist would say it is impossible.  But 1) If any one have ever has gotten HIV this way it hasn't been documentend so it remains a theorectical risk 2) she didn't have a sore did she? 3) you saw no blood 4) the chnaces she had HIV is less than 1%.

You would be the first person in history that could be documented that got HIV this way.  Have you ever heard of anyone finding a winning lotto ticket on the street?  I haven't, it is possible, but I am not going to count on it for my retirement! :)  Cheers!

I understand your point, not taken the wrog way. No worries.  I am just not sure if the Dr. is going to get down this far in the threads.  Also, I have read enough of his answer to know he will likely say it is so rare for anyone to get HIV through insertive oral there isn't much science to go with it. What we know is you don't get HIV from saliva so what he would likely say though it is POSSIBLE that someone has a mouthful of bleeding gums and slops a bunch of the blood in your uretha or festering open penis sore and IF that person with a mouthful of blood actually had HIV it would be POSSIBLE to get HIV that way.  It is also possible you will find a winning lotto ticket laying on the ground.

Thank you, Dr. H for allowing me to attempt my own risk assessment.  

My view is that I have full 100% ownership of my own sexual health.  Thus, it makes no sense to write you on this Forum without at least having tried to understand the issues and facts.

I don't think I could summarize the assessment of risk better than you have.  I would modify 1A to say there is zero risk of chlamydia; to my knowledge, penile infection with chlamydia has never been documented to be acquired by fellatio. 1A and 1B are correct.  Your assessment of no. 2 is accurate.

You have nothing to worry about and can move on.
