HPV questions

Hi Dear Doc,
  Greetings from Taiwan. First I'd like to thank you for doing this, it's a great favor to us worried minds. I donated twice due to technical difficulties with my PC, it's fine though, I'm sure it will be used well.

  Here's some background info, on 1/31 of this year I had protected, paid sex (but there was very brief genital to genital contact w/o penetration). Guilt had driven me crazy, so I have told my girlfriend, who's away from home studying in the US now. To make the long story short, we will give it another chance and see how it works out.

  I got worried a few days after the encounter, and went to a Uorologist and had urine and subsequently, blood, tested, I was immediately put on antiboitics (both injection into blood and medicine) ,for 2 weeks (yes,daily injection), I believe the Doc said something about white blood cells. After the 2 weeks, urine tested fine. I have not had sex with anyone since the 1/31 incidence. At 2 weeks, I had RT-PCR done and it tested negative for HIV.

  At 21th or 22th day after the incidence, I had some rash which was inching right around belly button (one just above and one beside it). No blisters. Not redness. Just itching. I was worried it might be Herpes, or could be as simple as the new shower gell. Maybe you could provide some insight on it. It did go away after a week or two. I dont think it is, but not sure..

  I am also concerned about HPV. From reading the HPV questions that I read in this website, I'm under the impression that you generally answer HPV related questions base on the assumption that one is infected with one type of HPV (for example, I quote: it takes around 11 months for HPV 16 to clear). HOW ABOUT INFECTION WITH MULTIPLE hpv TYPES? And assume the worst, multiple high-risk HPV types? I ask because I am very concerned about passing HPV to my girl, she will come home in mid-June, and penis cancer, to a lesser degree. No symptoms of warts so far, except a very small patch of dark skin on the glans, which two Doctors said was Chancroid, and one Doctor (at the most prestigeous hospital here) said "absolutely no need worry about it, it's just a minor skin problem." and gave me creams to apply at home. Since none of the 3 Doctors said it's a wart, I'm going to believe it's indeed not. I'll just use the cream and if a month passes and it's not better, go see a dematologist.

  Lastly, about 2-3 years ago I first noticed a white wart-like thing on my tongue, which I ignored until this month, I had it taken out by an oral surgent. The biopsy came back was a benign pappioloma (spell?). From what I understand, the types of HPV that cause warts aren't the same types that cause warts, correct? I do plan to have some other suspected ones all taken
out in the next visit. (I guess the bad news is that if it's caused by HPV, some other types may be in, too)

Sorry for the lengthy post, I'd greatly appreciate if you could address some of my concerns.

My girl have a genital HPV. Can i get HPV by touching
her genital skin with my hands? Or get it from her saliva.
Can i get HPV by touching something that an infected person touched with their hands?

Most of the information in suedeblueyes' comment about herpes and the original questioner's risk of HIV is just plain wrong.  And the moralistic tone is unacceptable. Neither distorted facts nor morizing are permitted on this forum.  Any further comments along these lines will be deleted without comment.

FIRST of all, what are you doing having UNPROTECTED sex in Thailand?!?  ESPECIALLY when you have a girlfriend? You should be worried about AIDS now, not Herpes or HPV!  Second, you CAN get genital herpes from a woman with a cold sore even if you DO have coldsores already!  In FACT, you can STILL get herpes simpex 2, genitally, even if you already have it!  So what's the difference?  And for the "forum" - are you an official counselor on this website? cuz if you are you are SOO under-accomplished, OBVIOUSLY!  People SHOULD be concerned about HPV. Oh, and that Chancroid you have - "unfaithful" - is probably syphillis - go check it out before you turn blind and mentally ill....FOR REAL!

You were clear and I understood.  It doesn't change my response or advice.  There is no chance of chancroid.  You almost certainly didn't catch anything you can transmit to your girlfriend and you can have sex with her any time you wish.

I'm so sorry, it appears that I didn't make myself clear. I had protected sex (virginal) with the sex worker, but there was a brief (a few seconds) moment of unprotected genital to genital contact.

I don't think you needed any testing at all.  You said you had "protected" paid sex w/o penetration, just brief genital to genital contact.  

Dr. HHH will be able to set your mind at ease, I'm sure.  Probably most sexually active people have been exposed to HPV anyway.  Its rarely a problem.  I think you are overreacting to your guilty feelings.

Sorry if it wasn't clear, but I have not had sex with anyone, since 1/31, and don't plan on having sex with my girlfriend (or anyone) until I feel it's safe to do so w/o putting her at risk (when will that be??)

Is it possible to get chancroid after the 1/31 incidence and so much antibiotics?

Forgot to mention: when I went to the Doctor a few days after the incidence, the Doctor told me I had UTI. That's why I was put on the antibiotics. I have lost faith in that Doctor, however, for using antibiotics injection for 2 weeks straight on me, which I later learned was probably not necessary (ya?)and I had to pay out of pocket.

You describe safe sex, with almost no risk of STD transmission.  My guess is your provider treated you in a preventive fashion, not because he diagnosed an STD.  (If you had abnormal white blood cells in your urine or other evidence of possible STD, you definitely did not catch it during the encounter you describe.  You wasted your money on HIV testing, especially the expensive PCR test.  You should not have requested it and your doctor should have advised you against such testing.  (But if you were frightened, you might have insisted and talked him into it.)

The rash you describe doesn't sound like herpes, but of course I cannot make a definite diagnosis or rule out any particular condition.  Whatever the rash is, you didn't catch it from the sex worker.

There might be a very small, theoretical risk of HPV by genital apposition, but too low to worry about; and there is no chance of catching multiple types during a single exposure.  If 3 doctors say the gential lesion isn't a wart, it isn't a wart.  I can't comment on your oral lesion of a few years ago.  You are correct that different HPV types cause warts versus cancer.

Bottom line:  You are much too worked up about this non-exposure which could not have resulted in any STD.  Forget about HPV, which 99% of the time doesn't matter.  Everybody gets it eventually and most of those never know it.  Stop further STD testing and just work on healing your relationship with your girlfriend.
