1. No, if you have HSV-1 you could get HSV-2 but not HSV-1.
2. You have negative blood tests and negative antibody tests. The evidence is strong that neither your sore nor your discomfort is not due to HSV.
3. Yes, if you were immunolgocially compromised you would have more pronounced signs and positve cultures
4. No, you would not need to double up on the dose.
As I said, there is little to suggest this is due to HSV. EWH
Thank you for responding to my post. I thought that you still could get hsv1 genitally even if you had it orally (I feel like many people have posted about it on the forums). Also, I feel like all of my 'symptoms' seem to fit in herpes. I get pain in my butt area (feels like muscle pain around my tail bone) that come and go and when it comes and goes my discharge is heavier and Im itchier. When I got the lesion on my rectal area it felt tingly/burning before it appeared. Also, the sore came when I was getting the itchy / discharge. However, it is un-hsv like that the sore is still there. I have a few questions:
1. Can you have two different genital hsv infections - like one on the vaginal area and one around/in the anus that flare up at different times?
2. Is there anyway that I could have hsv2 and my body not produce the antibodies ( I know that 5-10% of people dont make the antibodies) ?
3. If my body was "compromised" for some reason and not able to clear the hsv wouldnt I have more pronounced signs in my genital area?
4. I have felt a cold sore coming on and when I took the Valtrex it ended up not appearing - would it work better for one area and not the other or would I need to double up on the dosage if infected both orally and genitally?
I am going to stop taking the valtrex and see if it gets worse. I just feel like Im drowning in water - that nothing is helping or working and I have all the herpes "signs" but nothing is making sense. Thank you for taking the time to help me in whatever you can
Welcome to our Forum. As I read the first part of your post I was coming to the conclusion that the lesions and symptoms occurring at your vagina and around your rectum were not herpes (I will explain more below). Then I read your additional comment a few minutes later and that confirmed my suspicion- that the lesion you have noted, as well as the symptoms you report are not due to HSV-1 infection. When persons get HSV in one location of the body (i.e. your cold sores) they are no longer susceptible to infection with the same type of virus at other sites. Thus your positive antibody test is a reflection of your oral herpes infection (cold sores), not genital HSV-1.
In addition, I would also point out the even first episodes of genital herpes typically resolve over a period of about three weeks. Thus that your lesions have been present 6+ weeks is also inconsistent with HSV. Finally, your repeated negative cultures are also evidence that the lesion you are concerned about is not HSV.
As far as what the lesion might be, that is more difficult to say. If the doctors you have seen thus far are not sure, it is time to go and see a dermatologist who regularly evaluate patients genital lesions (not all do, you should ask when you make the appointment to make sure you get an answer from an experienced clinician).

oh and I do have hsv1 orally so if I would think that I have the antibodies already if I had it genitally too to prevent outbreaks
