Herpes Liklihood & Conflicting Info

Ok, aside from the Molluscum, I have some other symptoms I need advice on:

28 yr old female. Slept w/ 2 guys w/in 6 mo. period. 1st guy: Jan-Feb (~3x). 2nd guy: June (3x). After 1st guy, around March/April I got bad cramps and was itching/burning quite a bit. Was diagnosed w/ cervicitis, but tested Neg for Chlamyd., Gonn, and Trich, BV, and yeast. In May, took blood test & tested neg. for HSV 2; positive for HSV 1, but have had cold sores since 13yrs old.

After 2nd guy: Have cont. to itch/burn around opening, but even worse. Feel pinching sensations, & hurts at times in the opening. I have had what feels like hemorrhoids for like the past month. Skin btw vag. and rectum has been inflamed 4 couple days and burned when urinating. I have had 'hemorrhoid' feeling (blood when wiping sometimes as well) 4 many yrs, but never 4 this long of a stretch. The pain/burning/itching near vag. opening and at random times in random places causes me to think I have HSV. I've read that HSV can be mistaken for hemorrhoids, recurrent yeast infections, and can cause cervicitis.

Now, my gyno said he doesn't think it's herpes, b/c I already had the oral infection (Although I haven't gotten a blood test to rule out a new HSV2 infection from the 2nd guy). BUT, I went to see a derm. to treat the molluscum, and she said that that's NOT true, and I quote "I've had many patients who have HSV1 both orally and genitally). Dr. Hook stated, "Once you have HSV you cannot be re-infected with the same type of virus, even at a different site," on Aug. 23, 5:25pm. So, why the conflicting info?

The derm swabbed an inflammed area for HSV, results came back neg. Could she have not gotten the right area? Do my symptoms sound like genital HSV?

worried it's HSV cause: During this whole time I've been having back2back cold sores, and I'm even getting cold sore sensations in my eyes and I have a sore in my ear! (Saw a red patch on my eye lid that pinched and hurt like crzy lasted for a day).

I agree with the assumptions you mentioned.  My suggestion at this point would be to work with local care- warm baths and being sure to keep the region clean and dry after bathing (a hair dryer, set on the COOL settign is useful for drying the area).  EWH

Thanks for restating your question.  I'll try to clarify some of the conflict.

A person who has HSV-1 certainly can get HSV-2.  This happens all to regularly.  On the other hand, it is considered to be rather rare (certianly less than 5% of the time, for persons to get HSV simultaneously at more than one, non-contiguous locations.

As for your perineal irritation and sore, sorting this out is best done by culture or, better yet, PCR which tends to be a little bit more sensitive than culture.  HSV in this region can be mistaken for other problems (yeast, hemorrhoidal discomfort, etc) and other things can be mistaken for HSV. the fact that you have had symptoms for a month or more makes HSV unlikely. Even with initial episodes, unless you are otherwise immunosuppressed, HSV lesions typically resolve in 2 or, at most, 3 weeks.  This, combined with your negative culture makes the irritation unlikely to be herpes.

I hope this response is helpful to you.  If not, you may ask continue this thread so we can try again.EWH

and...continual outbreaks of oral herpes back to back to back during this time is NOT helping the situation either...

Thank you very much, Dr. Hook. I guess what is perplexing me is the fact that I've tested neg. for BV, yeast, etc. I'm just frustrated at this point b/c I KNOW I have symptoms of something, and I can't figure out what's causing them. I just went back to the derm. today, (after my post), to have her look at the lesion. She said she didn't see anything, and the anal irritation is probably due to something called fishers(?). I wish I would have pushed for her to take a culture, but I didn't. She seemed confident I don't have herpes and recommended I follow up with my gyno to do a full internal exam. She said there could be other things causing my symptoms. As for the lesion on the perineum, I guess I'll never know. Though blood tests are not recommended, I'm going to get one in a month anyways.

Thank you for the clarification, So, I guess what I can take from your answer (and my gyno's) is that, if I already have HSV1, most likely what I'm experiencing in my genital region is not due to HSV1. And, since one culture for genital HSV (1/2) was negative, I can probably assume I don't have HSV2 as well. Blood testing, I hope, will provide further evidence. Thank you very much for your reply, and sorry again for all that mess up top.

Dr. Hook,

I did not see your reply when I added those other posts to this thread. I have only had two threads; I thought each thread counted as a 'question.' My third 'question,' I assume came from the post where I was trying to point out a typo-I didn't think that counted as one of my 'questions,' my apologies.

Here is my question:

I have had itching, burning, stinging, and pinching sensations in my vaginal area near the opening for a month or two. Also, I've had what feels like hemorrhoids for this length of time (and for years) but never this long. The perineum has a tiny, red, open sore that stings when touched. My gyno says that he doesn't think I have herpes since I have had oral HSV. I tested negative for HSV2 blood test, but not after this most recent partner. I went to a derm, who swabbed an inflamed area, tests came back neg. for HSV. Does it sound like herpes? Could she have just not gotten the right spot? My derm said that you can catch HSV1 in other locations, and that she sees people on here with HSV1 in both areas all the time, that conflicts w/ statements that you have made. Just wondering why the conflicting info.

Hopefully you'll still answer my question. If you did, I would really appreciate it; I had no intentions of violating any terms and conditions, and sorry for the excessive detail.

We ask clients to ask no more than 2 questions on the Forums in any 6 month period to allow others access to get their questions addressed - this is your 3rd in less than 2 weeks.  In addition, we also ask that clients not exceed the space allowed to ask their questions.  A question too complex to be asked in the space allotted is too complex to provide useful information over the internet.  Please re-state your question, BRIEFLY and I will answer it.  EWH

Ok...ONE more thing to add, sorry for being excessive, BUT I just went to the bathroom..(bowel movement). First of all, the pain/sores around my anus was even worse, and bleeding was on the toilet paper, but that's been mentioned, so not so much a concern as the tiny, really tiny, circular sore on the perineum that when I touch it with my finger, it stung/burned like an open sore. It's not a bump, it's just as if the skin is open right there. (I took a mirror to see it, and it was just a little reddish dot) I am going to try to get into the doc. tomorrow, hopefully it doesnt go away, but I thought that info would be valuable to share as well.

Thank you.

*Also I've been getting random pinching sensations that last for a few seconds near opening and labia.

And, as far as the cold sores and eye pain/itching/red patches/etc. Could this be the result of an outside-in infection in these sites, or an inside-out infections. What I mean is, djo.harvard.edu states, "Occasionally, the virus does reactivate (stress!) and, instead of traveling back down the nerves to the mouth or nose, it goes to the eye causing the illness there." So, could my HSV1 have decided to take an expedition to every single nerve ending in my head??

Thank you very much for your time!!
