Painful urination theoretically could be a symptoms of herpes, but it is rare to get that symptom without also having obvious (i.e., easily visible) external genital lesions. Also, the risk of getting herpes from a single episode of fellatio is very low. Whether or not you have discharge has nothing to do with whether or not any infection you may have is curable or not. Most likely you have no infection of any kind. To answer your specific questions:
1) The IgM herpes test is unreliable and the results meaningless.
2) Your risk for HSV-2 is zero. There is a theoretical but miniscule risk of HSV-1.
3) As you say, I have addressed this many times. Will you feel more reassured if I say the standard answer also applies to you? Well, it does--so consider yourself reassured. There is no risk.
4) There is no need for a chlamydia test; chlamydia is never transmitted by oral sex. In theory, there is a risk for gonorrhea, but almost all gonorrhea causes discharge from the penis, not just painful urination, so you can be quite sure you don't have gonorrhea. But if you want to be tested, it can be done any time; the gonorrhea test is positive within 2 days of catching it. The chance of syphilis also is about zero, but if you want to have a reliable lab test, you need to wait for 6 weeks or so.
If still in doubt about any of this, consider a visit to your local health department's STD clinic for expert evaluation.

NGU is a urethritis which is non-specific (not Chlamydia or Gon).
I caught NGU from receiving one episode of oral sex. Exactly 8 days after the encounter, I started getting burning when urinating. Eventually I was diagnosed with NGU and given a two week course of antibiotics.
I would recommend you go to a clinic and get checked. NGU is very treatable so don't worry about it.

I'm still worried.
The pain while urinating does not go away.
I also get pain in the groin and lower abdomen.
Now I worry about HSV and HPV.
Does HPV cause pain while urination.
