Firstly would like to say what an excellent service you provide, and how many people you must make feel happier. I am a married man on the business trip, I have always been faithful with my wife, but I did one stupid mistake, very regret about it and this is THE ONLY exposure that I have had. I had sex with a sex worker about 5 weeks ago while at a massage parlor. She gave me oral sex (no ejaculation) with a condom then we had vaginal sex with same condom for about 1 minute till I ejaculated. I did not check the condom when finished but I am pretty sure it did not break (Since it is used for less than a minute)

Your exposure was low risk.  STDs are not frequently transmitted to the penis by fellatio; the risk of STD from condom-protected vaginal sex is similarly low; and the HIV risk was essentially zero for either exposure.  Your symtpoms don't particularly suggest HIV or other STD, you can rely completely on the negative test result you have had, and you probably can rely on the reassurance from your sexual health clinic visit.  (The term SHC suggests maybe you are in Australia or elsewhere in Australasia, yes?)  So I don't think I can provide any more reassurance than you already have had.  To answer your specific questions:

1) Very low.
2) Reasonably reliable, especially since your tests were negative.
3,4) Presumably this refers to your HIV test.  Your SHC guy was being conservative; it's closer to 90+% after 4 weeks and reaches 100% after 6-8 weeks.  But since your risk of getting HIV was virtually zero, it really doesn't matter much; you really didn't need HIV testing at all.  The gonorrhea and chlamydia tests were 100% reliable.
5) You can be confident you have nothing you can transmit sexually to your wife or anyone else.  And of course you can't give any STD to your kids (or anyone else) through nonsexual exposure.

My advice is to do nothing further and relax.  If you need still further reassurance, re-visit the SHC.

I think the doc will say that you had the safest sex possible and that you didn't need any testing in the first place so timing of the tests is not relevent. And that no one gets STDs from sex with condoms that do not fail. You should move on and enjoy your wife.

Thank you Professor, I really appreciate your advice and it make me feel alot better. I was in hell for the past 4 weeks.

You were right, i am in Australia and New Zealand.

Dear Professor

May i ask two quick questions if possible to clear my mind.

1) Have you ever see a case of having negative test at 3-4 weeks and turn to positve at 6weeks+.

2) As you mention with other posts, the highest risk of hiv transmission is 1/10. Could you please give one scenario of such case for us all to understand?.

The highest risk sex is probably a homosexual man receiving unprotected anal sex from multiple partners or from an anonymous partner.  

Your exposure was very low risk.

I've been with my boyfriend for a year and a few months, we have been having sex for about 10 months and in about the 5th month I developed a bug bite on my inner thigh ( right next to my vagina), I kept scratching it and it became a rash and I still have it. Could it be an STD?
My sex history is lost my virginity to a virgin and now this guy is my second sex partner.
His story is a little different, he has had sex with a few girls and one girl cheated on him with other guys without using a condom because she was on the pill.
When I looked at his pelvic area and it seems like he also has the rash but on the inside of his thighs.
Any advice?
