Liver concerns

Dr.

I wrote you about 18 months ago with an HIV concern regarding an Asian massage.  No sex, but I vaginally inserted my finger and a couple of minutes later I went to the shower and stuck my finger in the showerhead.  Water/fluids splashed in my eye.  You stated my HIV risk was void.  A few weeks after that incident I went to the doctor with an ache in the liver area.  I also explained my situation and he did nothing.  About 4 weeks later I had my annual lab work with normal LFT's with a slightly low total protien.  I experience sever gastro problems for the next 9 months and was even refered to a GI whom had no concerns for the liver and thought all my problems were caused be my gallbladder removal.  One year later I had elevated ALT and AST followed with an ultra sound that showed possible fatty liver.  My questions are - 1.  What is the risk of Hep B in this situation?  2. Would normal ALT 4 weeks after swollen liver indicated no hepatitis infection with this situation?  3.  With lingering issues I am concerned about chronic Hep B.  Does modern testing help determine how long one may have had this?  4.  I'll will be seeing my primary physcician in a few weeks.  I will request Hep screen.  Any additional questions I should be asking?

Thanks for your service.

Most hepatitis is not an STD and I am not a hepatitis expert.  You need to speak with your own provider, or if you want online advice, you might try the American Liver Foundation website (http://www.liverfoundation.org/).  (I don't vouch for ALF and do not know whether they accept questions like yours.)

Mild abnormalities of liver function combined with 'fatty liver' suggests an alcohol problem--but I don't think fatty liver can be diagnosed without a biopsy.  As I said, I'm not an expert, but if a gastroenterologist wasn't concerned, you seem to have no worries.

You could not have acquired viral hepatitis during the sexual event you describe.  Even if you turn out to have chronic HBV or HCV, you got it someplace else.

Good luck-- HHH, MD

One more question?  I am assuming my 11month negative HIV test is confirming even if there was a coinfeciton?

Sorry, I missed the comment about the ultrasound and fatty liver.

Of course the negative tests prove you weren't infected.  Why are you even on this forum asking the question?
