Massage - handjob

Dear Doctor,

I had a full body massage, and 10-15 min. Handjob in a chinese parlor in the US 21 days ago. She was always clothed. However, I didn't check her hands for any sores or blisters.

For the past 21 days, I regularly checked my fever, and body. Had no symptoms or blisters, sores whatsoever.

I got tested with 4th Generation DUO HIV test, and RPR Syphilis test at Day 19, from labcorp. Both negative.

Since I didn't have any blisters or sores by Day 21, I believe I can eliminate Herpes. For HSV, I didn't get tested.

My questions are:
1) I know the duo test is not conclusive at 19 days, hoever what accuracy would you guess for this test done at 19 days? Do I need further testing based on this event?
2) How about RPR at 19 days, and chance of Syphilis from this event?
3) Do I need HSV testing based on this event?
4) Is my reasoning for eliminating Herpes reasonable?

Take my comments above at face value, stop overthinking it, and stop searching the web. You're seeing scare stories.  Hand-genital contact transmission of STDs is pretty much an urban myth:  theoretically possible but extremely rare.

That's my last comment.  Do your best to move on.

If she has stds and there are sores in her hands, direct hand-penis contact will not transmit std?why?
I read that some stds can be transmited skin to skin and genital is susceptible .

Yup. That's what I would do, if I were in your circumstances.

So, since this is a no risk event, can I basically forget about this and continue having unprotected sex with my wife?

Welcome to the forum.  Thanks for your question.

The exposure you describe carried no risk, for all practical purposes, for HIV, herpes, or any other STD.  These infections are not transmitted by hand-genital contact.  If this is your only potential risk, I would have recommended against any tests.  To your specific questions:

1) A negative duo test at 19 days is around 90% reliable.  As noted, I would not have recommended any testing, and therefore see no need to do another test.  On the other hand, having started down that path, if for reassurance you need a conclusive negative result, have another duo test at 4 weeks or more after the evnt.

2) There is no risk of syphilis.  But RPR at 19 days is meaningless -- much too soon.  For conclusive results, test at 6+ weeks.

3) I recommend against HSV testing.

4) Asymptomatic HSV infections occur, so absence of symptoms is not, in itself, conclusive evidence against infection.  But you weren't at risk, so I wouldn't worry about it.

Best wishes and happy holidays--  HHH, MD
