NGU and Urethitis

I went in to get tested for my partners peace of mind recently and was surprised to find my grahm stain came up with a high white blood cell count. The physician deemed it NGU and prescribed 1g Azithromycin for my partner and I. My current partner is monogamous and was a virgin before intercourse with me. I will have the test results back for the standard STD screenings in a few days. I should aslo mention that I occasionally get cold sores and have been identified positive for HSV1. My partner has never had a cold sore outbreak.

I was tested about 1.5 years ago and have had 4 female sexual partners since that time. 3 of which involved protected vaginal sex, 1 unprotected monogamous and tested partner. 2 of the 4 partners performed oral sex.

I had not had any prolonged symptoms of an STD or ureter infection. There was a brief period of a few days where a slight burning sensation was felt during urination. I assumed this was dehydration since it was not prolonged. This instance occurred after having protected sex with my current partner early in our relationship. We have since had unprotected sex on numerous occasions relying on birth control as a contraceptive.

I have been tested 3 other times in the past 6. 2 were due to a burning sensation during urination and a slight discharge. All previous screenings came up negative for chlamydia, gonorrhea, etc. but an antibiotic was prescribed and cleared up the issue(s)

I have a few questions about my situation...

1) is it possible for my partner to have a PID before becoming sexually active? We have had sex 3 times in our relationship during menses if is helpful

2) Is it possible that I have reoccurring Urethritis, even with time spans that far apart?

3) The past instances of an infection while testing negative for chlamydia and gonorrhea happened after having protected sex. I thought this may be an allergic reaction to latex condoms and I have since switched to a polyisoprene condom. Just prior to getting tested recently I used a latex condom since no other option was present. I do not consciously buy condoms with spermicide, but they are all lubricated. It it possible that I am having an allergic reaction to the latex or lubricant causing an infection or temporary inflammation of the ureter, causing my white blood cell count to go up?

4) My current partner and I have been having un protected sex for ~ 3months. What are the chances of her developing a PID in this time span from an NGU? Infertility is a large concern for her and I. She was given 1g Azithromycin for treatment when I was diagnosed.

Sounds like you may be dealing with persistent NGU and the 1g Azithro should clear you right up.  The puzzling thing here is that you claim this happened after protected sex, so your assumption that it may have something to do with the lube in the condom may be correct.  Did you receive any unprotected oral prior to intercourse?  

1)  You partner is not at risk of PID.  She should be fine if she is not experiencing any symptoms.  It is possible to have PID prior to any sexual activity, as there have been documented cases of virgin teenagers contracting PID.  As you probably know, it can be caused by bacteria other than the run of the mill STD bacteria.  However, this is rare.

2)  Yes, you could have persistent NGU.  You said you have been treated for it multiple times in the past, so something is bringing on urethritis.  Since you've tested negtive for NG and CT, the causative organism is unknown and may never be known.  In these cases it's very difficult to pin point the reason, but always try to document the events prior to an episode of NGU, and hopefully you will figure out what activities to avoid or what precautions you should take.

3)  Yes, it could have been the lube on the condom that brought on this recent bout of NGU.  It could be chemical or bacterial, depending on what kind of fourplay you had prior to intercourse.   If there was never an exchange of bodily fluids (vaginal or saliva), then it's most likely chemically induced.  

4)  Your partner most likely does not have PID.  She would be noticing some kind of symptoms by now if she had it, but the fact that you were the only person she ever had sex with and that your NGU doesn't sound like it's caused by bacteria, it leads me to believe that she has zero risk of PID.

if she was a virgin before you, no she didn't have pid.

many things can cause a + gram stain. at this point wait for your std test results to come back and go from there.  many "normal" bacteria can cause urinary infections also.   should you test + for gonorrhea or chlamydia, your  partner will need properly treated. Since this is being detected fairly early, not a worry for pid developing.

you were only treated for chlamydia at this point. should your symptoms reoccur despite negative std tests ( we'll think positively ), return to be seen for further testing and treatment.
