Doc,

Thanks for your dedication to people like me that have crazy questions!  Off to it...

--Unprotected oral (me receiving) with female worker
--no symptoms besides pink eye two weeks later
--second week in Jan. took 8 day supply of amoxicillian for unrelated sinus problem
--during third week, took z-pack.  Still no symptoms of anything
--during course of z-pack, had sex with regular girlfriend
--two days later, she had pink eye and a yeast infection, which went away with monostat
--she took z-pack, cleared everything up

March--
--gf started spotting in between periods (she had just started new birth control).
--this went on for two cycles.  She got off birth control, spotting stopped
--everything back to normal

IN BETWEEN SEX WITH MY GF AND NOW, SHE HAS TAKEN 3 Z-PACKS AND A 10 DAY COURSE OF DOXY-75MGS X2 DAILY FOR UNRELATED ISSUES)

I HAVE TESTED NEGATIVE FOR CHLAMYDIA AND GONORRHEA 3 TIMES SINCE JAN INCIDENT--ONCE IN MARCH, ONCE IN MAY, AND ONCE IN JULY.  NO OTHER EXPOSURES SINCE

Aug. 1st
--gf starts having cramping after her period and feels a little nauseas.

QUESTIONS
1) What are the odds of me catching chlamydia/gonorrhea from the unprotected oral that lasted 2 mins.?
2)  Would the antibiotics that were taken take care of the problem/risk, if there even was one?
3)  Would my gf develop PID from this incident?
4)  If she had gon./chlam., would it even show up in a test now since she has taken the different antibiotics?
5) Do I have anything to worry about?

Thank you in advance doc.

If your partner (not you) is concerned about her symptoms, she should see her h health care provider.  I have no other comments or advice.

just to follow up:

1) Regular female partner just finished her period a week ago.  Now she feels nausea and has a little leg pain.  Should I be concerned about PID?
2)  She just took (3 weeks ago) a 5 day supply of Levaquin, 750mg each.  Would this clear any long term chlam/gono infection if in fact she had it?
3)  I know I do not have anything now--concern is that I had it without symptoms (oral sex scare) and my antibiotics cleared it--but before I finished my full course, she caught something
4)  To reiterate, she over the course of 7 months has had 3 full 5-day regimine zpacks (1.5g total each), 10 day doxy course, 75mg 2x daily and 5 days 750mg levaquin.  Would this for sure clear anything up?

I know my risk was considered "low" being oral sex, especially without symptoms.  How low are we talking?  Again, I took amoxicillan and 2 z-packs as well, right after the encounter.  Please help doc, last question.  Just really freaking.  Got on Wellbutrin for anxiety.  Thank you for your consideration.

Almost all men once in a while notice a bit of discharge when straining at stool.  The pressure of feces against the prostate gland, combined with straining, expresses some prostate gland secretions. It's normal and doesn't change my overall opinion and advice.

Taking azithromycin or not won't have any impact on your partner's health.  You have nothing you can transmit to her, so the question is irrelevant.

I won't have any further comments in this thread.

In April, approx. 3+months after incident, I was on the toilet during a bowel movement.  I never saw anything actually come out of my penis during urination, but I saw a very small amount of discharge in the toilet.  This was the one and only time I ever saw anything.  I immediately went and was tested, and results for gon. / chlam. were both negative (urine test at health department lab-results took 1 week).  I never ever experienced any sort of discharge in my underwear or anywhere else, and it never hurt to urinate.  Could this be something left over from an initial infection that cleared up with the meds?  If I was taking amoxicillan/z-pack and had sex with my partner, would this prevent any infection?  I will leave you alone now.  I apologize for the questions, but would sincerely appreciate a response to this, it will make things clear to me.  I didnt know if this one-time discharge during a bowel movement would be significant, even though I tested negative.  Just made me think that it was something left over even though I never showed any symptoms early on.  Thanks Dr. Handsfield.

If you "know this is irrational", what do you hope to gain?  Women get pelvic cramps all the time and there is no reason to suspect she has PID or anything related to your event several months ago.  Your guilt is the only problem here.  If her provider diagnoses PID, then worry about the possible source.  But there's really no chance.

I know that I am not infected now--my concern is that I was infected, infected her, I never showed symptoms, and the meds she took did not eradicate the infection in her.  Just me. The cramps worry me given that they are showing up 7 months later.  Is this something to worry about?  Should I be concerned?  Would the z-pack and doxy have wiped out any gon. / chlam. infection that she might have had?  Sorry doc, I know this is irrational.  Last question.  Promise.  Just need to know if I could have infected her.  Thanks in advance.

You have been tested and negative 3 times for an exposure back in January and are still worried you might be infected?  Get that OCD taken care of.  It appears to be the main health problem of concern.

1) Chlamydia of the mouth and throat is rare, and therefore chlamydia is never acquired by receiving a BJ, or at least too rare to worry about.  Gonorrhea almost never occurs without getting classical symptoms of pus dripping from the penis and often painful urination as well.  Given the nature of exposure, your negative tests, and the treatment, there is simply no chance you have either infection (unless you have been reexposed since the event last January).

2) Yes, the antibiotics would have cleared either infection.

3) Your gf can't be infected by you if you aren't infected yourself.  And you were not.

4) Irrelevant.

5) No.

Good luck--  HHH, MD
