Over concern from act?

Doctors,

First I would like to say thank you for the great job you all do on this forum. I am one of many who have been terrified by doomsday information on the Internet. Thankfully I found your website. I have read numerous responses by you similar to my experience but want answers to my specific event in order to hopefully put this behind me.

On 11/5/12 I had protected oral and protected vaginal sex with an escort. She was adamant to use a condom, not that I didn't want to but she pulled one out immediately.  She even went as far as to wipe my penis with a towel before placing the condom on. These facts make me feel as though she was more worried about receiving an std from me but then I think we'll maybe she was trying to not infect me since she had an infection. Not sure she would go to those lengths to protect me, someone she doesn't know.  First was protected vaginal sex, changed condom followed by protected oral and then protected vaginal sex again. Total time was about 20-30 minutes. I am married and am completely sickened by why I did this in the first place. I have had no symptoms of anything. On 12/24/12, about 42 days out, I woke up and noticed a single red bump in my pubic region. Middle of the ways up on the left side. No pain to it, only a little feeling when pressed. There were 2 hairs growing from the middle. I pulled both hairs and squeezed the bump. A little white puss, not much, came out followed with a little bleeding. Spot began to disappear and was gone by 5-6 days. If I run my finger over the spot now, there is the slightest raised area, I wouldn't be able to even notice had I not known the bump had been there.  my genitals so haven't paid attention.

About 5 days ago a much leas pronounced red spot appeared at the top left of my pubic region, with a hair in the middle, also pulled the hair. Yesterday I had another smaller red bump appear on the left side, near the edge of my pubic region, also a hair growing out of the middle, I have not pulled the hair. None of these spots are blistered and had I not had this sex act, I would not thought much at all about these spots. No clustering of any sort. Honestly look like pimples. I believe I have probably had these many times before but have never had a reason to check my genitals so haven't paid attention. I do not shave my pubic region.

My wife and I were virgins when we married so this is the only possible exposure I could have possibly had in my life. I feel somewhat confident that most all stds would have made themselves apparent by now but am worried about HSV2. Could these bumps be an initial outbreak?

Also:

1.  What are my chances of transmission of HSV2 from this one act I described.
2.  Could this be my initial outbreak. Not typical sores and also have read where you have noted the pubic region is not typical for an outbreak. Is this area especially not typical for the initial outbreak.
3. If you were me, would you be worried you were infected and are asymptomatic.
4.  Would you have any worries to engage in unprotected sex with your wife.
5. Would you suggest testing.

I go back and forth in feeling confident I'm clean but have setbacks sometimes when reading some people talk about being asymptomatic or very mild symptoms that go unnoticed. I have had no obvious or typical signs of anything other than what I mention above.

Again, thank you very much for your honest and non-doomsday help to all of us.

I am sorry. I tried to break ini 2 posts.  Act is as follows:
11/5/12 protected oral and vaginal sex with escort. She insisted on condom use, I did as well. Makes me think she is more worried about catching std from me. No symptoms until 12/24/12. Single red bump in middle of pubic region with 2 hairs in the middles. I pulled the hairs. A little white puss squeezed out and bleed a little. No blister, looks more like a pimple. Just a single red bump. Completely gone after 5 days. Very slight raise in skin is there but I wouldn't even notice it had there not been a bump there. 4 days ago a less pronounced smaller red bump with hair showed at top of pubic region, with hair in middle. Another on the left edge of pubic region last night. All have had hairs growing from the middle. I do not shave there and the hairs are normal length.

Thank you very much

Also wanted to add that this initial bump appeared about 42 days out from the possible exposure, even if she was infected. Also this is the only other person I have had sex with other than my wife. Neither of us have had sex with other people other than my stupid decision. This would be the only possible exposure to HSV2 for me in my life.

Thanks for your editting.  You have nothing to worry about.

The exposure that you describe was very, very low risk.  Most CSWs do not have STDs and those who insist on condom use are typically doing so to protect themselves, not their clients.  Further, condom protected sex is safe sex with virtually no risk for STD acquisition.

The lesions you describe in your pelvic area appeared long after any STD, including herpes would be expected to appear and occurred in a location where STDs are uncommon.  On the other hand, what you describe is a great description of folliculitis.  Folliculitis occurs when the pores in your skin get plugged.  This allows that normal bacteria that are present on just about every-one's skin to grow and cause what is essentially a pimple.  Some of these just appear as red bumps which may or may not be  tender while others go on to actually form pimples  That you see hairs coming out of some or most of the lesions pretty much assures me that this is folliculitis.  Folliculitis lesions are best if they are not squeezed.  They can be both treated and prevented with soap and water.  Use of a washcloth or loofah sponge helps as well.

1.  What are my chances of transmission of HSV2 from this one act I described.
See above- almost zero.

2.  Could this be my initial outbreak. Not typical sores and also have read where you have noted the pubic region is not typical for an outbreak. Is this area especially not typical for the initial outbreak.
This is not HSV.

3. If you were me, would you be worried you were infected and are asymptomatic.
No, I would not be worried.

4.  Would you have any worries to engage in unprotected sex with your wife.
No, you should have no concerns about having unprotected sex with your wife.

5. Would you suggest testing.
No, testing is a waste of time and money.

I hope my comments are helpful.  EWH

I have read many posts on folliculitis.  I do work out a good bit so this could drive this I suppose.  I believe I have probably had many spots like these before but never paid much attention until now as I knew I had opened myself to a risk. The first single bump which was a little more pronounced immediately caught my attention. My first thought was Merry Christmas, I have herpes.  Your comments are greatly helpful. My main issue has been guilt. A persons mind can be the most dangerous thing. Again I thank you for your forum and hope others can gain education on items like this as I have.

Dr.

Just another thought out of curiosity. Since you say I have been well outside the period that herpes would appear, why the big concern of some people who have been out many months from exposure that their symptoms are lying dormant and will appear later?  Just curious.

Thanks

You ask a good question. I think the stigma associated with herpes, misguided as it is, has fueled a lot of mis-informtation and mis-interpretations about herpes.  The internet tends to fuel tis.  Not sure if this is the entire reason but it is certainly part.  EWH
