Pain in testicles and urethrea

I have had this recurring problem between my girlfriend and myself.
I contacted this germ(do not know what it is) a couple of years ago.
The symptoms initially are pain in urethrea,testicle pain,back and leg pain,frequent urination,tooth pain(sometimes)
Stomach cramps.heart pain
I had some success with doxycycline before but the doctors are not willing to give the medicine to me till they know for sure what's causing this.
If anyone knows what this is so I can get the right test for it please let me know

if you have extra protein in your urine, perhaps it's time for the urologist.

My regular doctor doesn't have a clue
The only thing they seem to get is extra protein in the urine
But that result  seems to be slowly coming down

the infectious disease specialist probably was not a good place to start.  have you had a full work up by your regular doctor?

I have been to infectious disease with no help.
They gave me doxycyline once and it did help  
but doctors wont give me another dose till they know what it is.
Do you think a urologist will be of more help?
Its more of all these symptoms and pain and headaches.
