Penile bruise, should I be concerned

I'm wondering if anyone has experienced something like a bruise on their penis. I have a 1 x 1/2" gray/purple spot on right side if penis behind head (over vein). Not sure if I should worry or normal.

Background: About 2 weeks ago I had some rougher than average anal sex with a partner I just met. I was protected for all penetration but may have bumped around a bit prior.

About 10d after this I experienced blood in my semen and sensitive/swollen right testicle. I went to doctor to get checked out and they indicated STD was unlikely but with level of sensitivity in teste (epididymitis) they gave me treatment of 2x antibiotics and STI test (still pending results).

Since then I have been searching my penis oh too often and have noticed a ~1inch x 1/2inch dark colored area (gray/purple) on right side of penis behind head (over vein). It is sensitive to touch (combination of bruise feel and chaffed penis feel).

The additional information doesn't change my opinions or advice.  We don't examine posted clinical photos on this fourm -- and anyway discoloration of any sort is not an STD symptoms, so its appearance would not be likely to change anything.  I'll be happy to comment further after you have been reexamined professionally and/or a specific diagnosis is made.

Doc,

Thanks for the quick response. A few points of clarification:

- anal sex, I was giving not receiving (along with vaginal, but anal mor rigorous). Not sure if this matters
- symptoms showed up ~10d after the sex
- also symptoms arrived day after very heavy drinking, which though some liver issues may explain some symptoms...not all
- antibiotics seeming to help the swelling of testicles, but still a bit sore and taint still sore to hard touch

Picture if bruise, slight discoloration behind head. In case I may be misdiagnosis gas bruise

Welcome to the forum. Thanks for your question.

Responding first to the title, before reading anything else:  Probably no worries about STD.  No STD would show up as a bruise (or anything that looks like a bruise) on the penis or anywhere else.  Whether or not you should be worried, however, depends on other (non-STD) risks, injuries, etc.

Now having read the question, this is a puzzling case and I can't come up with a good explanation.  My guess is that the bruising is related in some way to the problems with hematospermia (blood in semen) and testicular pain 10 days earlier.  It makes me wonder whether there was internal bleeding (maybe trauma from the vigorous anal sex??), and that blood in the tissues leached gradually down into your penis.

The pain in the "bruise" area doesn't concern me.  Blood in the tissues, whatever the origin, is irritating and probably the cause of the tenderness.

I have to wonder whether you had epididymitis -- testicular infection -- that explained the testicle pain plus hematospermia.  Gonorrhea can cause these things, although pretty rarely; maybe chlamydia, although I've never heard of chlamydia causing hematospermia.  And with protected anal sex, both are unlikely.  Which maybe comes back to the trauma possibility?

You should return to the doctor who evaluated and treated the earlier problem and show and discuss the penile bruise.  Or if that doctor wasn't a urologist, such a specialist would make sense.

Sorry I can't be more helpful.  If and when a clear diagnosis emerges, I'll be interested to hear about it.

Best wishes--  HHH, MD
