Please Help! complications after treatment for chlamydia

Well I'm a 19 year old heterosexual male and  about 2 months ago I have unprotected sex with a girl I've known for many years. Then about 2 weeks later I recieved oral sex and the day after the oral sex I started to suffer from pain while urinating and an itching sensation in the tip of my penis. About a week later I was diagnosed with chlamydia and recieved the antibiotic azithromycin. The symptoms went away after about a week. Now about a month and a half later I began suffering from an acing pain in my testicals and lower abdomen pain. I thought this might have been epipididmyths so I went to the doctor where I was given a 1g dose of azithromycin and took another urine analysis. Now about a week after taking the 1g dose I still have symptoms. Not the acing pain but a tingling in my testicals and an overall abnormal feeling in my groin. I just recieved the results from the urine analysis and there was blood in my urine. I was referrered to a urologist but can't get in for a few weeks. Any help would be much appreciated because it is horrible living with this. Could my testicals be dying?
