Syphilis is transmitted by direct sexual contact with an infectious lesion. The risk of syphilis from unprotected oral sexual contact is very, very low and should not concern you.

I repeat, you should not worry about syphilis. The doxycycline you took would have prevented/cured syphilis in the unlikely event that you were exposed through the exposure you describe. Your mouth ulcer was not syphilis.
When syphilis occurs, typically the ulcer appears about three weeks after sex and the blood tes for syphilis becomes positve less than 4 days after the ulcer appears. EWH
Last question doctor how long the syphilis symptom showing after exposure and how long to take the chancre to heal, my ulcer is heal now so that is good sign ? Also my penis don't have symptoms so far, after 4 weeks if my blood test result come out negative I shouldn't worry to much about syphilis ? I doubt that your doctor has the tools to do a swab test for syphilis. I also doubt that the lump you mention is syphilis- if it is, your blood test should be positive. You and your doctor should look for other causes of the lump. EWH

Welcome to our Forum. I'll be pleased to comment. The exposure that you mention was very low risk. Most commercial sex workers do not have STIs and if she did, most exposures to infected partners do not lead to infection. Further, condom protected sex is safe sex was little or no realistic risk for STIs, including HIV.
That leaves your receipt of unprotected oral sex. Few STIs are acquired from receipt of oral sex. There are no cases of HIV which have been proven to have been acquired through oral sex. The major risks from receipt of oral sex are gonococcal or non-gonococcal urethritis (NGU). The doxycycline you have taken would cure or prevent NGU, most gonorrhea and syphilis.
If you did not perform oral sex on your partner your mouth ulcer was not due to anything you acquired from the exposure you have described. Receipt of oral sex would not lead to symptoms in your mouth. I suspect that the swelling and ulcer you describe are due to something other than the exposure you have described. In your situation, it would not be syphilis.
Testing for HIV with a test for antibodies will detect 90% of recent infections at 4 weeks (28 days), 95% at 6 weeks, and all recent infections 8 weeks after exposure. A 4th generation, combination HIV antigen/antibody test will detect all recent infections 4 weeks after exposure. Having taken doxycycline, you do not need to take a syphilis blood test but if you had syphilis, most persons would have positive tests 30 days after exposure.
