Protected Sex, Testicle Pain

I had protected vaginal sex with a CSW in Singapore 4 weeks ago. There was no oral, but she did give me a covered hand job prior to the protected sex. The penetration lasted maybe 2 minutes before I ejaculated. The condom was fully intact when I withdrew, although I did notice that the semen was coming up the shaft of my penis near to the top of the condom indicating that there was not enough room in the tip to hold all the semen. The condom was provided by the CSW, so I am worried about its effectiveness, although there was no breakage.

In the last week I have developed bladder pain and discomfort in my left testicle. There are no other symptoms at all (no painful urination, no discharge, nothing of that sort). I went to the doctor and rather than test for gonorrhea and chlamydia, he prescribed me 1500 grams of Zithromax and gave me an injection in my buttocks (can't remember the name of the drug) on the assumption this is what it was. I told him it was protected sex, and only happened once for no more than 2 minutes, and he said it was still possible to catch these diseases because there could have been a microscopic tear or defect in the condom.

My questions are:

1. if this is true, and I did actually catch chlamydia or gonorrhea, is it possible that I caught HIV too, even though I used a condom properly (i.e. no breakage)? I know have to wait for the 3 month window to be tested, but I am worried about passing it on to my regular partner.

2. Could my testicle and bladder pain be caused by something other than an STD? The doctor said the pain would go away within 48 hours. Its been 24 so far, and I still feel pain.

3. I had protected sex with my regular partner 2 weeks after I had it with the CSW and before I took the medication for chlamydia and gonorrhea. Could I have passed these diseases on to her, even though we used protection the one time we did it?
