Questions following unprotected oral

Hello Dr.

On July 30, 2010 I had a sexual encounter with a high dollar escort. She performed unprotected oral sex on me and we also had protected vaginal sex. After this encounter I continued my life as normal, including having unprotected sex with my wife.

On or around 8/10/10 my wife complained of feeling symptoms of UTI (burning when peeing) a couple of times when she urinated. The painful urination stopped for her the next day, but she went to see her Dr. who tested her for UTI. She was negative for the test, and was not treated for anything. The Dr. suggested it was due to dehydration and encouraged her to drink more water. Since then, she has had no complaints of painful urination.

After this I began feeling a tingling sensation at the tip of my penis with no pain on urination or discharge. This sensation was off and on, and I believe it probably was mental due to the guilt I was feeling. After around a week I stopped noticing the sensation.

Around 8/24/10 I began to notice pain in my left testicle. I had a problem with testicular pain in the same testicle back in 2008 and was treated. The pain went away after treatment. The pain I felt this past month was very similar, and has gone away after I have started wearing briefs rather than boxers.

Today (9/6/2010), I noticed a clear sticky discharge from my penis. This is the first time I have noticed it. I suppose it's possible I have had the discharge for a while without my noticing, but this is unlikely I have checked very frequently, even obsessively, since the exposure.

My question to you is do these symptoms sound like I may have an STD? I am mainly worried about having transmitted something to my wife, though she has not complained of any symptoms. I have been tested, but the ARNP at the clinic was unwilling to provide any conjecture about what I may or may not have and the likelihood of STD. The results from the test has not come back, so any information you could provide would be appreciated.

Welcome to our Forum  You have done the right thing by going to get tested.  While, unlike the ARNP you saw, I am willing to engage in conjecture (after all, that is what you have paid for), the best answer to your questions will come from testing.  Your genital exposure was unprotected and STDs are rather uncommon after receipt of oral sex.  I anticipate that your tests will be negative and if so, that ultimately you will wind up attributing each of these problems to coincidence, mixed perhaps with a substantial serving of guilt.  With that as introduction, let's work through your symptoms one-by-one.  

Intermittent tingling.  Sometimes after an exposure that, in retrospect, one wishes they had not had, persons tend to examine themselves and be far more attuned to genital sensations than in periods when they are not concerned.  This in turn leads to noticing what turn out to be normal sensations that might have been not noticed or ignored at other times.  The description of these heightened sensations is often described as a "tingling" sensation.  Further, the intermittent nature of the symptoms would be atypical of an STD.

Testicular discomfort.  In the absence of penile infection, this is unlikely to be an STD and more likely to be a recurrence of a prior difficulty.  Having had a problem in your left testicle in the past you are likely somewhat more susceptible to recurrences.

Clear discharge.  While this could be a problem and should it persist, I would have it evaluated, my guess is that this is normal genital discharge. Such discharges are a bit more common when one strains at stool or in the morning on arising.  

Bottom line, the exposure you describe was rather low risk.  It is unlikely that you acquired an STD but if you had, your tests will help to diagnose what is going on.  If the tests are negative however, I would work to move on and not worry about the exposure you described.  I hope my comments are helpful to you. You are the one who has decide whether or not you need testing. I have told you that your risk is low and it is.

Please stop examining and squeezing yourself.  This can cause irritation on its own and does not in any way help to address your concerns.  EWH

Today I have still noticed discharge but it takes a lot of squeezing and working to produce only a very slight amount. How much discharge is normal? Should I still be evaluated even though the discharge doesn't appear without some serious manipulation of the shaft.
