STD / HIV Risk - Advice please

Hi,
On 30th June I visited a prostitute & received oral & engaged in vaginal sex (both protected). The prostitute concerned I've visited adhoc over a few yrs & works in the UK but is Czech. 2 days later I had protected sex with my g/f and received unprotected oral. 7 days later from my prostitute visit I had a white powdery discharge which dried and could be brushed away. The discharge happened once & I visited my doctor a few days later who advised he couldn't explain it as the sex was protected and the discharge was not ongoing but the risk of contracting something was slim to none. He also stated the discharge didn't appear to be typical of Chlamydia or gonorreah. I took the medication but I had continued discomfort in my penis that hasn't gone away. 10 days after the medication finished I had vaginal sex with my g/f and the condom broke. 7 / 8 weeks from my encounter with the prostitute I've developed a rash across my chest, back, face and the tops of my arms/shoulder area. The rash is a combo of flat blotches, spots with heads and raised bumps - red in colour (ARS rash?). I haven't had any flu like symptoms, I'm a little light headed/dizzy but this could be anxiety. I also had a small amount of discharge after having intercourse 2 weeks ago with my g/f, again one off and very small. I'm confused; I thought I'd done the right thing by protecting myself however I am experiencing symptoms. I've contacted the parlour where the escort works and they advised the girls are tested regularly and have to provide proof of this or they're not allowed to work. I'm not sure how much I can trust this information?
It's fair to say I am stressed over this. I recently (officially) separated from my wife and living arrangements have been a challenge etc so life has been far from simple. I was stupid in visiting the escort and things have got more serious with my g/f hence I am worried forher and me. I'm unsure whether to accept I was protected and move on or what to do!

My urine tests came back negative for any stds from the clinic. I didn't have a HIV test done for the reasons previously indicated. Are you comfortable that I move on given my protected exposure and negative test for other sti. Thank you for all your help

1.  Yes, I anticipate that your tests will be negative..
2.  They send them to the lab for testing. Urine can be tested for gonorrrhea and chalmydia.
3.  Yes, it will be OK to continue relations with your partner.


I continue to doubt that that you have gottten an STD. I will have nothing more to say until we hear of your test results.  EWH

Hi Doctor,

I went to the clinic this morning and had a swab and urine test done. Both were clear initially although they send them away to the lab and I get the official results in 7-10 days. I did not have any bloods taken - I'm not that great with needles! and based on your initial feedback re the exposure I had I didn't think it required. The symptoms are persisting on an off. A few questions

1.) Would you still expect the std results to come back clear?

2.) Why do they send them away, what is the mid stream urine test for?

3.) Do you still think it ok to continue sexual relations with my partner, she has shown no symptoms that I'm aware of.

FYI - I've been given a potassium solution for UTI and Cystitis as a precautionary measure to flush out the system. The doctor didn't say it was either of those things but just advised to take it for a week and then if it continues to ask my doctor for a urologist referral.

I still can't help but think it's too coincidental to not be an STD!?

I'm sorry, I ahve nothing to add. You and I have come to different conclusions.

Please let us all know what your STD evaluation shows.  I would be most suprised if it were positive and remain inconcerned about the possibility of HIV from the exposure you describe. EWH

Further to the previous correspondence I am trying to move on but the discomfort in my penis remains and I have had another small discharge mid morning this morning. Again it has dried in my underwear and can be brushed away ie: non sticky. I am really concerned and have made an appt at the std clinic for next week. The discomfort in my penis and this discharge suggests something isnt right and I can only assume an std is the cause. This then highlights further concern over the rash that appeared after 7 - 8 weeks that could be ARS. The rash has cleared up after 2 - 3 weeks.

I'm very worried that an std has been caught despite the protected sex and HIV is also a factor due to the rash. I'm so confused and concerned - please help!

Thanks for the clarification.  The discharge that you describe really does not fit as an STD related discharge.  It's intermittent nature and appearance sound, if anythng, as though it could be ejaculate or seminal fluid rather than infectious in origin.

As far as ARS is concerned, I would urge you to set your concerns aside.  The rash has been there too long and does not sound like the rash of HIV.  Finally, I do think it would be OK for you to resume unprotected relations with your regular sex partner. EWH

Thank you for your prompt response it's much appreciated. Apologies as I didn't explain correctly - the initial discharge was powdery in appearance but wet. It dried to a bright white and once dry could be rubbed away easily ie: it was non sticky or residual. With the subsequent small discharge a couple of weeks after medication should I be concerned. Discharge of any sort is alarming albeit its not constant. Does it sound like an STD discharge.

With regard to the rash does it even sound like an ARS rash? I note your comments re  the timeline. It also seems to have changed in the last couple of weeks. It has been consistent on my chest but has changed on my back and neck/facial area. The backache I'm also assuming is unrelated as it wouldn't have lasted that long?

If you're confident from what I've described that the risk was not there I will work it through and move on. I assume also that I would be ok to continue sexual relations with my g/f who has had no symptoms as far as I'm aware.

Welcome to the Forum.  I agree with your doctor.  With protected sex, your risk of having any STD, including HIV is virtually zero.  I have no good explanation for a "dry, powdery" urethral discharge. In fact, it is difficult to understand how dry material would come from the penis.  Are you sure it is not from some other source.

As for your rash, I suspect this is an untimely coincidence.  The rash of early HIV typically occurs 2-4 weeks following exposure and for it to appear at 7-8 weeks would be virtually unheard of.  If you feel you must, you could have an HIV test which will provide reliable results for you at this time.  Personally however, I see no reason for even testing in relationship to the protected encounter that you describe.

I suspect your backache is likewise unrelated.  Sometimes after an exposure that, in retrospect, one wishes they had not had, persons tend to examine themselves and be far more attuned to genital sensations than in periods when they are not concerned.  This in turn leads to noticing what turn out to be normal sensations that might have been not noticed or ignored at other times.  Perhaps this was a contributor to your situation.  

My advice would be to work through these various issues with your own doctor if they trouble you beyond your concerns that they might be STDs.  EWH
