STD/HSV risk from massage encounter

I'm 45 and married with no history of STDs.  About six days ago, I had a massage with a happy ending from a sex worker.  (I've had these before years ago with no issues).  There was no insertive sex or oral, but there was some kissing on the mouth and she did put some saliva on her finger and briefly touched my penis (on the shaft).  She also briefly straddled me without underwear- I don't think there was genital on genital contact but there might have been fleeting contact with my penis and my groin area.      

A few days later I developed a burning sensation in (on?) my penis.  Feels a little like a UTI but sometimes on the outside.  Sometimes feels like a little tickle inside and like something is crawling out of me.   Haven't noticed any sores, spots, redness or anything like that on my penis.  A day or two after the encounter, I developed a canker sore inside my mouth that has resolved.  Today, I woke up and noticed a small red blotch in the crack of my butt about an inch from my anus.  Doesn't seem like a blister or pimple or a raised bump.  Still have mild burning in my penis and groin area.  Sometimes I feel the burning in my legs, eyes, lips. I saw a doctor yesterday and he took a urine sample-- didn't really think STDs were an issue but spent maybe 5 minutes with me and didn't examine my groin (as mentioned, there would have been no sores or anything like that to see).  

I have had similar experiences with burning in my penis in the past related to sex (never with happy endings).  Seems like it could be anxiety but don't really think so.  I also have had kidney stones and prostatitis in the past (but not recently).
  
My questions are:

1.  Could this be HSV (either oral or genital) or any other STD?  I know from reading the forum that the answer is generally not from manual sex, but there was the saliva on the finger and straddling- not sure if that's a real risk.

2.  Could I have previously been infected with HSV and this is just a flare up from the stress of this experience?  I've never had an outbreak as far as I know but all the information out there is confusing as to being infected without being symptomatic (or just missing the symptoms).  

3.  Could HSV or any other STD only be on the inside of the penis (which would cause the burning) but never on the outside?

4.  How long should I wait before asking for further tests and avoiding sex with my partners?

5.  Could this be just a regular old cold virus or other infection?

Thanks.  On #3, would it possible never to have any externally visible outbreaks and only internal.  Not even sure what it would feel like.
