STD Symptoms Related to Oral Sex

Dear Doctor:

   I am a 27 year old male, and approximately 5 weeks ago I received unprotected oral sex from a female escort. Within the matter of a week I began experiencing burning with urination, an itchy penis, and testicles. I proceeded to go to a medical clinic, where I received the standard treatment for chlamydia/gonorhea, with Azithromycin and Macrobid for the pain.

   One week later my symptoms continued, so the clinic referred me to a urologist. My urologist ran urine tests, a KUB (ultrasound), all which came back negative. Treated me with Doxyxycline and Pyridium.

  I am experiencing horrible symptoms even to this date. My penis becomes inflamed, especially when exposed to heat, and severe burning/itching after urination. Even Motrin 800 only provides mild relief. Other symptoms include swollen neck nodes, anal discomfort/burning, and fatigue.

   I realize this is not the HIV forum, but nevertheless I freaked out and went for a test last week Wednesday at a local health clinic. The test, which was administered orally, came back negative, yet I am still very worried. Could this be Herpes, HIV, or some other illness? What sort of tests should I seek, and what can I do to help relieve my horrible symptoms.

   Any help you can provide will me most appreciated.

No STD causes the sorts of symptoms you describe.  You're right -- these symptoms aren't connected with the sexual exposure you're concerned about.  You have a cold or allergies.

Dear Doctor,

   I have a follow up question due to some symptoms I have been experiencing. For the past week I have been dealing with a sore throat, swollen neck nodes, and headaches. Ive tried my usual OTC allergy meds and pain killers, but to no avail. Could this be STD related? Note: my previous symptoms with the penile pain/burning urination/itchiness has greatly diminished.
    I realize that this probably is not connected, but I would rather be safe than sorry. Any information or advice would be of great help.

Dear Dr. Handsfield,

   Thank you for your quick response. The medical worker at a local health clinic did state that there was a possibility of Herpes, and that I should be tested nevertheless just for piece of mind. I think I will, and whatever the outcome I will be sure to let you know.
    I appreciate all that you do on here. Will try to relax, though my symptoms (whether psychological or not) may continue. One thing I should note: my urologist treated me for prostatis, which could also be an issue for me. I work an office job, and sitting on my behind is my 9-5.
    I have just been a basket case worried about STDs, fearful I had HIV or Herpes or something). Thanks so much again for your input.

Welcome to the forum.  Thanks for your question.

I would like to help, but I cannot -- except to reassure you that nothing dangerous is going on.  You have a rather common problem -- unexplained symptoms after a new sexual exposure.  The various tests you have had are highly reliable, and the treatments you received would have eradicated all known STDs that are known to cause such symptoms.  Neither herpes, HPV, nor any other STD is responsible, and treatment for them makes no differnece in symptoms.

Genitally focused anxiety is the usual explanation -- not that anxiety alone crates physical symptoms, but it certainly can magnify normal body sensations to painful levels, or increase awareness of otherwise minor symptoms that otherwise would be ignored or forgotten.

What we know with some certainty is that nobody with symptoms like yours ever later develops serious health problems -- no infertility, cancer, or other difficulties.  Similarly, men's sex partners don't develop any problems.  So whatever is going on in such cases, it is not apparently due to any infection.  And experience shows that once affected men come to understand that nothing serious is going on, the symptoms often fade away.

So my advice is to do your best to move on.  Don't have any more antibiotic therapy, and continue or resume sex with your regular partner(s).
