Seeking advice. (STDs/Herpes concern)

Hi Dr

M28, Uncircumcised. I'm scared.

My sexual history:

Before 2012: 18 protected vaginal/anal, Unprotected fellatio, bare cunilingus/rimming with cheap CSW. I’d 3 unnoticed broken condoms with CSWs. Tested -ve for STDs/Herpes I/II many times in 2012 and moved on.

New exposures after –ve tests:

2013:
Very brief Unprotected sex twice with non-virgin friend of unknown status and 12 Protected vaginal/oral with cheap CSWs, deep bare cunilingus/rimming(with oral injuries), French Kiss & bare genital frottage in UK. Also only one unprotected fellatio with CSW.  Some masturbation with vaginal fluids also. Some CSWs had red rashes/bumps on labia/vagina/thigh.  I closely scrutinized my penis for some time after each sex.

Only symptoms after sex involving unprotected fellatio and protected sex was that after 14 days (had one last protected sex on this day with CSW) I saw a white bump on mid-penis shaft which I popped. Some off white/yellowish pus came out. It’d scabbed the next day but I picked at it for 1-2 days in anxiety & made big wound of 0.5cm which scabbed/healed with PVP-I (Betadine) gel in max 6 days total.

After one more week I saw another white bump on base of shaft which I popped. White/yellowish pus came out.  I scratched badly and made a big wound of 1cm which healed with Betadine gel in 9 days max.  Wound was scabbing but I always kept reopening it in these 9 days of the healing period it during masturbation or by picking at it.

No hair coming out of both wounds.  

I felt these symptoms were not herpes related since they were not typical of it as per my research and given my really low risk exposures.

Tested –ve for all STDs at a private clinic 4 weeks after last sex.  No herpes test this time.  I also read that herpes blood test is not so reliable always.

Getting married in few weeks.

Can I have UNPROTECTED sex with her without fear of infecting her with STIs or HERPES?  Or do I need to seek more testing?

Thank you doctor.  Was just anxious. Perhaps due to my lack of sex education.

Putting this behind me forever now and moving on with my life with my wife. Getting back to the marriage preparations!!!

You are overthinking this.  No change in my assessment or advice.  If the condom covered the head of your penis you were protected.  EWH

On the last occasion I also had one pimple on scrotum/base of shaft on which the CSW had performed unprotected oral and I also used a penis vibrator ring I found there which touched with the pimple as well as fresh genital area shaving wounds (30 min old).  All this with the piercings thing above may be causing holes in condom, the condom slipping a bit and all.. please consider and tell me last thing

I am just anxious.  I will not ask anything after you answer. Just want to put it behind me.  As said negative for all STIs but No Herpes test done.

also some of the CSWs in 2013 had oral piercings and long fingernails and used same condom for oral and vaginal. One even inserted her long nails inside condom before putting it on with her mouth. Condom did not appear broken at end of sex though.  Maybe small holes in it??

ANd in most of my protected sexes with CSWs in this year the condom slipped halfway or more during sex but penis head was always covered all the way during penetration.

just these last worry.  Let me know if it changes anything.

Dr I have one last question please.  In most of my protected sex in this year the condom slipped halfway or more during sex but penis head was always covered all the way during penetration.  I assumed this still is protected sex but still its casting a doubt in my mind.

Does this change anything in your assessment or advice?

Thank you doctor.  I will hence have no further STD test and no Herpes test as you suggested.  Thank you for allowing me to move on with my life and saving my future marriage.

Yes, I did include herpes in my assessment and advice.  the lesions you describe on your penis do not suggest HSV at all.  EWH

Just to add, I was freshly shaved and had nicks/cuts on shaft/scrotum on the day I got unprotected oral from CSW.  Please read my above comment about my understanding of your answer.

I will make sure that she gets her HPV vaccine.  

I will seek no additional testing, which I suppose you also mean for Herpes also since I did not get it after my exposures/symptoms in 2013.  I imply that your answer included Herpes also when you said that I can have unprotected sex with my wife without STIs fear.

Should I be wrong in understanding it, please inform me.

Welcome to the Forum.  My answer will be much shorter than your question.  Yes, you can comfortably have unprotected sex with your wife without fear that you have an STI which you could transmit to her.  Your tests for gonorrhea, chlamydia, NSU, syphilis, hepatitis and HIV are all conclusive and should be believed

The only exception is HPV.  Given your history the odds are high that you have HPV.  You probably have it (but cannot test- there are no HPV tests recommended for men), like most sexually active persons and if your partner has had other partners, she probably does too.  She should get the HPV vaccine, as it is recommended for most women and is highly effective in preventing acquisition of new HPV infections.  

Other than making sure that your partner has had the HPV vaccine, there is no reason for concern.  There is no need for additional testing.  EWH

All the CSWs were Eastern European/Hunagarian or Romanian.

Test done four weeks after last sex was: HIV Duo, Gonorrhoea (oral swab also), Syphilis, NSU, Chlamydia, HCV, HBV, Urine tests.

Iv decided to be monogamous and settle down and have kids but not before I can be confident that I will not infect my wife with STIs/HERPES!

I will be happy to clarify anything further.
