Swollen meatus only after ejaculation

Dear Doc, I had protected sex with a girl. Next day I had itching on my testicles. I doused baby powder all over my genital area (literally all over) to control the itching. To clean, I used LACTACYD (Lactic Acid, Lactoserum) skin cleanser on genitals and area.Days later I had protected sex and unprotected oral sex with another girl.  I continued the baby powder and LACTACYD cleanser.
Itching on my scrotum still persisted and moved toward my shaft. I had dry skin on my scrotum and midway up my shaft.  I went to the doc and he gave me Clotrimazole Cream USP, 1% for Athletes Foot, stating it was a fungus, no tests were taken. I have been applying the cream for 8 days and itching on my scrotum has reduced significantly, almost cease to exist. But, after I masturbate my meatus is swollen and extremely red. On the meatus, swelling tends to decrease after 15/30 minutes, only redness remains. Morning there is no swelling on the meatus (looks normal) this only happens after I masturbate. When I masturbate I use Vaseline/KY Jelly and I stay at the bottom of my shaft,I dont irritate my head/meatus, but after I'm done, it's still swollen, no friction has touched it. The doc couldn't give me any clear answers on this(he didn't know). The meatus does not itch nor hurt when I urinate.

I have had no tests for STD's.
Have you heard of STD's that cause symptoms like this? (Herpes?)The swelling on my meatus only appears after I masturbate, it doesn't itch/hurt to urinate. I have HPV, but i'v never had any warts on my penis, only thighs/surrounding areas.
No condom was used during oral sex, anyway that the fungus was moved to my shaft and meatus, and that's why I'm having swelling/redness on my meatus after masturbation. Could there be fungus in my urethra?
Should I have my urethra swabbed for bacterial? Or should wait this out, and see if it gets better?
Could this be irritation from the baby powder/LACTACYD cleanser I was applying? Should I stop masturbating?
Thank you

No STD causes symptoms like you describe.  I suppose there is some slight chance that someone with urethritis (gonorrhea, chlamydia, or NGU) could have swelling or irritation of the meatus that would be more prominent after ejaculation than at other times, but I have never heard of it or seen such a case.  As to other (non STD) causes, my opinion would be even more speculative, and I would wonder whether you are just looking more closely than before and noticing normal physiology that previously you weren't aware of.  Certainly increased redness and prominence of the meatus could be a normal response to sexual arousal and orgasm.

As to the more general genita/scrotal irritation, no STD causes such symptoms.  Fungal infections, allergy, and other skin conditions all are possible and I cannot speculate.  As a general principle, when an unknown skin irritation is present, it is best to stop all treatments for a while and see whether the problem goes away, gets worse, or doesn't change.  Therefore, I suggest you stop the powder/cleanser.

If either problem persists, return to your health care provider.  As suggested above, to be completely safe, perhaps you should have a urine gonorrhea/chlamydia test.  But I expect the results to be negative and don't think any of your symptoms has anything to do with your sexual exposures.

Good luck--  HHH, MD
