Transmission (rubbing with breasts), Treatment, Etc.

First, thanks for this forum and all of your help (to so many).

My encounter involves a trip to a nasty (unclean) strip club, where I received a hand-job with her saliva being directly applied to my penis (by her spitting onto my penis and into her hand) multiple times.  She also placed my penis between her breasts a couple of times for several minutes of rubbing.

Based on what I have read here, the hand job and saliva should not cause any STD.  My initial questions are: (1.) Is the risk zero? (2.) Does the amount of saliva impact the risk level? (3.) Does the prolonged breast contact change the risk level? (4.) My penis probably had some recent chaffing and would that increase any STD risks?

Last week, and approximately 3 weeks after the encounter, I briefly stopped into a walk-in clinic and a doctor looked at  my urine and performed a stick test on the sample.  Based on those two items, he did not believe that I had any infection.  Is this a reliable test for STDS like gonoorhea, chlamydia, NGU, etc?  In other words, if the doctor doesn't see any infection with that test, does that mean I don't have any urinary tract infectdions of any sort (including STDs)?

Since the doctor knew I was worried and feeling some symptoms, he provided me with a single dose of azithromycin, which I took late last week.  A couple of medicine related questions: (1.)Is there any harm in taking the medication if there were no infections? (2.) Since taking the single does I have felt and still sometimes feel possibly more sympotms of a urinary tract infection (chlaymdia, gonorrhea, NGU) - is this normal and/or a possible side effect? (3.) Does it make sense to get tested to comfirm that the medicine worked and that I have no STDs(for both my specific situation and in general) - if so, when should such a test be done (to avoid a false negative)?

Finally, and a little after three (3) weeks from the encounter, I noticed a small red dot/bump on my shaft.  Could this be an STD?  I'm assuming it's not a genital wart (HPV) since there was no intercourse (oral, vaginal, anal) and I'm inclined to believe that's the case for genital herpes too.  Then I wondered if the strippers breasts could have any impact on this (maybe something was on the breasts or some residue from another guy's penis that had recently been placed there)?  What do your think?  Also, shouldn't the herpes have appeared sooner, or not necessarily?

I still have some discomfort and tingling in and around my penis.  Also, more frequent urination and desire to urinate. Is there any reason for me to be concerned about STDs?  Also, should I be concerned about infecting my partner with unprotected sex?

Sorry for the long message, and I'll appreciate your response.  I have been worried about everything and, if possible, I wanted to get answers to each item.

Thank you.

There are an awful lot of questions there.  But the most important information I can give is that there is no way you could have acquried HIV, gonorrhea, chlamydia, or any other STD from the exposure you describe--regardless of the amount of saliva involved, prior penile chafing, etc.  They call them sexually transmitted because you have to have sex to get or transmit them--and the contact you describe doesn't count as sex for purposes of transmission.  Therefore, unless you have had other exposures that actually confer some risk, you can be 100% certain your negative STD tests are accurate.  (If you cannot be infected, a negative test by definition is 100% accurate.)  And of course that also answers your question about transmitting to your regular partner:  you can't transmit something you don't have.

I cannot comment on your red dot/bump or your urinary symptoms; they too cannot be due to the exposure you describe.  Brief courses of antibiotic won't do any harm in an uninfected person.

I suggest you put this entire event out of your mind and stop seeking care or advice about it.  Of course if your symptoms persist, follow up with a health care provider--but with confidence that whatever is causing them, it has nothing to do with the nont-exposure event you describe.

Good luck-- HHH, MD

Thank you for the quick feedback and, again, sorry for all of the questions (though two brief questions follow).

Another worry I have had was the strippers saliva getting into my urinary tract and causing some problems.

Based on your response, it sounds as though this should be of no concern - correct?

Is that because drops of saliva do not transmit STDs - even HSV-1?

Thanks again and I have no more questions.
