Trich Maybe?

One week ago, I noticed a burning pain during urination which has continued. The pain is significantly worse (severe) in the morning and during the first urination following ejaculation. In fact, I can still the feel the pain for a bit afterward. The pain also seems to be worse at night. During the middle of the day (assuming no ejaculation), the burning is mild. Additionally, a few days after this started, I noticed swelling and tenderness of the inguinal lymph nodes.Also, the urethral opening seems a bit inflammed after urination or ejaculation (very red and tender).

I also experienced some buring during urination a few months ago, albeit with slightly different symptoms -  at that time, I had some mild flank pain and urgency to urinate combined with not feeling great in general (no inguinal node swelling)

However, this time the node swelling concerned me so I went to a local doctor here in St. Martin. The doctor took a urine sample and stated that it didn't "look" like an STD. He "thinks" I might have a prostate problem and sent me home with a 7-day course of Cipro. I have been taking the Cipro for 4 days and have not realized any relief. He said to come back in 5 days if the symptoms don't get better and he will send to a lab for testing (not an easy task here).

My questions:

1)Could the urine test the doctor conducted in his office have failed to detect the presence of an STD, possibly trichomoniasis? If so, what are the chances of that being the case?

2)Do these symptoms sound like trich or another STD to you?

3)Since the lab results here will likely take a while, should I ask the doctor to treat me pre-emptively for trich with a combination of Azithromycin and Metronidazole? Is it safe to take these antibiotics with Cipro? I would like to have this resolved asap, even if its just an outside chance that these antibiotics will be effective. There is not a urologist on the island and I really would like to get rid of these symptoms asap.

Thanks for the additonal information.  

You still did not provide the sexual history I requested.

The fact that your tenderness is improved may be a good sign and suggest that this is a slowly ressolving infection (prostate infections can do this).  At this point, there is not much to suggest that this is an STI but in the absence of an exam and tests we would all just be guessing.  My advice is to return to the Dr. who has already seen you and seek re-evaluaton and perhaps testing- there are many possibe causes of symptoms of this sort.  EWH

Thanks doctor.

The burning is generally only bad in the morning or for the first urination post ejaculation, with a very slight burning during urination for the rest of the day (barely noticeable).

The lymph nodes are swollen on both sides (they are moveable and not hard when I press them) but the tenderness, which was never very bad, has seemed to disappear but im not sure.

The Cipro has provided no relief

The doctor did not do a rectal exam.

I was curious to know if this was indeed an STD, UIT or other bacterial infection of the urethra, what are the chances it wouldn't have indicated as such on the urine test the doctor ran?

Welcome to our Forum.  I will try to help.  To do this I will first need to ask a few additional questions.  

Can you tell me your sexual history,.  In the month or so before your problems began, did you have sex with a new partner or a partner who might have an STI?

Also, about your swollen lymph nodes- are they swollen on one side of both sides (left and right) of your body?

Finally, did the doctor do a rectal exam of your prostate?  This is a helpful test of the prostate gland and can provide useful information depending on whether the prostate is tender or not.

Finally, is the burning on urination always present when you void or does it come and go.  Is there a discharge associated with it?  Have the antibiotics you are taking made any difference at all?

With regard to your specific questions, trich seems unlikely. In men trich infections often cause few symptoms or, if they do, the symptoms are mild.  I doubt that this is trich and would not suggest treatment.  I see no reason for taking the medications you suggest.
