Very Concerned about Gential(genital) Herpies(herpes)

Dear Doctor, about 3 weeks ago the lower shaft of my penis began to develop a red itchy and oval shaped red spot right in the middle about 5 mm in diameter.  Before and after the red spot developed I was masterbating (masturbating) very frequently (4x day for about 4 days) without lubrication due to stress at work so I thought it was just irritation (leading up to that point I was masterbating (masturbating) 2x a day mostly).  Although the red area is slightly raised and appears to have a tiny crater or two, I never saw any puss or fluid or blisters develop, although it's possible I could have popped any potential blisters without realizing it while masterbating (masturbating) because I continued even though I was a little itchy.  I aslo chew tobacco and thought that I might have gotten tobacco on my hands and then on my shaft while masterbating (masturbating).  I also sweat very much and have some dry flakeyness around my penis head just before the shaft.  The red spot to date has never been sore or painful and the itchyness stopped after only the first 2-3 days I noticed it.  However, 23 days later, a very red spot still remains which has never scabbed and I have not even touched the area for 16 days.  This itchy feeling I had the first 2-3 days has only happended once before that I can remember about 2-3 years ago and it was further up the shaft almost at the tip on the underside (this time its right on my circumsition point over a vein on the underside) and looked more like a pimple or white head.  I went to get myself checked out the first time (2-3 years ago) at Planned Parenthood and they said it was nothing which is also why assumed this time was nothing.  Since the red spot has remained this time I went to my Derm. exactly one week ago and he suspected genital herpies (herpes) from the symptoms I described but wasn't positive.  I've been putting some kind of ointment (Bactroban I think) he prescribed 3x daily for a week now and the sore has not really changed at all.  I also went for an HSV 1 and 2 Select test a week ago and the results say HSV 1 positive and HSV 2 negative.  I have not had any cold sores as an adult but may have as a child.  I've been with the same sexual partner for the past 6 years but strayed twice (My partner just got tested and came up HSV 1 negative and HSV 2 negative along with negative results for every other STD).  So, if I contracted genital HSV 1 in the past 6 years, the only time I can think of was with one individual over two years ago and another about 4 years ago. I had unprotected oral sex only with both of them and call them high risk because they were call girls.  I was drunk and do not remember if there were any cold sores on them when this happened.

So, from what I describe above do you think I have genital HSV 1?  Is it possible I just irritated myself and burned it from excesive friction?  Is it very likely I got it from two unprotected oral encounters over two and four years ago?

I look forward to your feedback.  Thank you very much.

From your description, the penile rash doesn't sound typical for herpes, and most people with genital herpes due to HSV-1 do not have recurrent outbreaks.  Also, herpes never lasts for more than 1-2 weeks; a persistent rash or red spot for more then 3 weeks is unlikely to be herpes.  But I cannot come to any more specific conclusions.  I'm a bit surprised your dermatologist didn't suggest a specific diagnosis if the lesion still was present.  I suggest following up with your dermatologist if the lesion persists; or if it recurs, see him/her immediately, within a day or to.  In the meantime, it would be good to stop irritating it with repeated, frequent friction--i.e., lay off the masturbation for a week or two.

Good luck-- HHH, MD

When I said dry flaky area just before the shaft, I meant the head.

Can herpes lesions last longer than 1-2 weeks if irritated by a lot of masterbation (masturbation) while the blisters are forming?  That's what I fear I've done but you have given me some hope that herpes is not what's going on due to the duration.

Thanks so much for your help.

Herpes lesions would be unlikely to last so long even with repeated friction or other irritation.

I have been doing a lot of research about the HSV-1 transmitted from oral to anothers genital area. Mainly because my husband had a cold sore and not thinking about it we continued our normal bedroom pleasures just two days later I noticed and felt enough pain to ask him to stop while we were having sex. It was then I noticed the sores. I had two one one side of the lab and one on the other. The trooper I am, I am just trying to wait it out thinking hot baths and triple antibiotic ointment applied twice daily (which seams to help subdue pain so far) is enough to wait out the duration with out drugs. After doing reasurch this doesnt look so great as I read that it could reoccour and it could last 3 weeks!! Not wanting to be so much of a trooper anymore, although your statement DR. HHH you say the duration is signitifanitly less and doesnt ofter re-occur. I need to know if your statement was veig or on target. ??? where do I post new questions? Thank You for your time and patients.
