What is it?

I had unprotected sex just over 4 weeks with someone new. During sex she started bleeding. I asked her if she was on her period and she said no. I then took a shower and scrubbed extremely well to clean off. About a week later it was painful when I urinated. I went to the doctor and got the chlamydia and gonorrhea test and said they would call me if it came back positive. I received no call. The urinalysis said I had urethritis. The doctor gave me the pill for gonorrhea(forget the name) and the antibiotic doxycycline as a precaution and said the pain would go away if it was a bacterial infection. 5 days later it was gone and I finished the treatment. Towards the end of the antibiotics I started to notice tiny red bumps and a small cut just under the head of my penis. There is no itching but I do feel a minor burning sensation. I only feel it part time. I don't know if its because I am worried about it or because it is on the back side of my penis and lays against my scrotum so doesn't get much air or because I stretch it to look at it. I have not felt sick, had pains in my legs or had any blisters and it is not sensitive to touch. I don't know if its herpes or a yeast infection or what. Since the first sign of redness it has been about a week. If it is a yeast infection would tioconazole ointment 6.5% work for sure. The girls says she doesn't have anything. She then went to the doctor and hadn't heard anything back either. Your insight would be greatly appreciated.

It sounds like you acquired nongonococcal urethritis (NGU); or conceivably chlamydia or gonorrhea, but presumably those tests were negative since apparently you weren't called with a positive test result.  You were treated the right way; doxycycline is one of the drugs of choice for NGU.

Your subsequent symptoms don't suggest an STD.  They are quite typical for a yeast infection, especially if you are uncircumcized and the bumps and 'cut' are under the foreskin.  Also, as you suggest, skin areas that don't get much air during the day are susceptible to fungal infection and other irritation.  Many women carry yeasts in their vaginas, without symptoms of a yeast infection; and doxycycline and other antibiotics can cause such organisms to grow and cause inflammation.  Any of the 'azole' creams probably will clear it up, so it's OK to try tioconazole.  If that doesn't promptly clear things up, visit your health care provider.

Your partner needs to be tested for STDs and should be treated with doxycycline (or azithromycin) even if your and her tests are negative.

Good luck--  HHH, MD

It is definitely starting to look better since about 20 hours ago. How long should I use the ointment and how many times a day? I truly appreciate you help. This is a great site.

Thanks for the thanks about the forum.

Continue to use the antifungal cream for 3-4 days after the rash has cleared up.
