What is my risk from this oral sex encounter?

Hello Doctor,
Thank you so much for taking the time to help me.
I am a 20 year old, healthy woman. I have had no previous STDS and have only engaged in oral sex. I am still a virgin. What are the chances that I would contract the following STDS from unprotected cunnilingus? (Receiving oral from a man)

1) Gonorrhea & Chlamydia (I’m not worried about gonorrhea of the throat. Should I be?)

2) HPV & Genital Warts

3) HSV 1 & 2

4) PID

5) Mycoplasma Genitalium

6) Any other STD

7) Risks of cervical cancer?


The reason I ask is because I had an oral encounter about 5 months ago and have been having weird symptoms ever since. Could be that I’ve worked myself up so much or something not related to a STD, but one never knows.

About 3 weeks after the encounter I woke up with itching and burning vagina, no discharge, and no pain upon urination. Used an off brand Monistat cream and it went away and haven’t had trouble with itching or burning since. Never had a yeast infection before so I hope that’s what it was. Would it have gone away if it wasn’t? I have also been urinating frequently (with no pain, burning or itching) just going between 5-9 times a day, sometimes more if I think about how much I’m going. And lately have had dark brown/black vaginal discharge and some spotting blood. Sometimes its heavy blood so I think it may be period but I don’t have cramps or anything. Really does not have a smell and there is no pain. I feel fine, no temp, nothing.

I also got tested for Syphilis, HSV 1 & 2, HIV with Western Blot, Hepatitis, (all through blood) and Chlamydia and Gonorrhea (through urine NAA Test) about 2 months after the incident. Everything negative- with Syphilis and HIV being “non reactive.” (I had just gotten of amoxicillin a week prior to testing. Could that have caused results to be false?)  Then Chlamydia and Gonorrhea again a month after that. Still negative. Are the tests reliable? Should I get tested again?

Stop overthinking it!  You came here for reassurance and I gave it.  Accept it and move on with your life.  There was little or no risk, and the test results show for sure that you didn't catch anything.  Stop looking for more information about it either on MedHelp or anywhere else on line.  You're obviously hyper-anxious, and that means you'll be drawn to bad news stuff that doesn't apply to you.  Let it go.

Okay, thank you Doctor. Just one last question.

I read this from a post that one of the community members posted, I have one comment.

"STD's are not transmitted from saliva but from bacteria so when a man give a woman oral sex it is basically on the outside and with the lips and tongue. For men receiving they are putting the penis inside of the mouth where the bacteria is. Same thing about performing oral sex."

My question is: Is there more risk that this guy put his tongue up into my vagina and not just on the outside? Not sure how far it was but was definately up there. Sorry for the graphic-ness.

1) Your test results prove you aren't carrying either virus.  2) Yes, you can disregard warts and HPV.  3) Trich is impossible from oral sex.

Continue to work with your own doctor and/or PP about your symptoms.  Stop worrying about STD until and unless you start having intercourse.  When that time comes, select your partners with care and use condoms and don't worry so much about STDs.  The tone of your questions suggests you are far more fearful of STDs than is warranted.

Thank you very much Dr. Handsfield, that does make me feel much better. I would just like to clarify a few things.

1.) Since I have not had any visible lesions that would be caused by herpes, and my herpes 1 & 2 tests were negative when tested 2 months after exposure, should I be at all concerened about carrying the virus? I have read where you can get herpes on your cervix. Is that likely in my situation?

2.) So are genital warts also unlikely or completely impossible?

3.) Could Trich be a possibility?

I would also like to add that I did end up going to my Dr. while I was having the frequent urination and that's when she put me on Amoxocillin for a "UTI" which I really don't think I had. (Forgot to mention the "yeast infection" a month earlier) She said there were WBCs in my urine and that there was an infection. What kind of infection could have this been? That's when I thought I had a STD and got tested. So definately not STD related? It's just so weird that this all happened after this incident!!!!

I do plan on going to my Dr. again when I get the money, I got a job at a toy store (yes, during christmas) to pay for the hefty bill. My university does not have a health center, and the nearest Planned Parenthood is pretty far away from me. I just hope this is nothing serious or to worry about :( Hopefully just a hormonal imbalance or something.

Welcome to the STD forum.  Thanks for researching your question before asking it.  You also have had accurate responses on the STD community forum.  I can confirm all you have learned.

In general, oral sex is safe sex -- not entirely risk free, but with much less STD risk than vaginal or anal sex.  And in the oral sex arena, the large majority of STD transmissions occur from fellatio (oral-penile contact).  Cunnilingus carries very little risk for any STD.  In addition, STD infections are uncommon in heterosexual men and women.  Therefore, even if STDs were easily transmitted by cunnilingus, such transmission would remain rare.

Of the STDs listed in your numbered list, the only significant possibility of oral to genital transmission by cunnilingus is herpes due to HSV-1.  All the others occur extremely rarely, if ever.  Since there is no known HPV risk, there can be no cervical cancer risk.

As for your genital symptoms, they were mostly compatible with a yeast infection.  However, I am concerned that you didn't get professionally evaluated.  Self-treatment for presumed yeast is fine in women who are experienced with genital yeast infections and are having the same symptoms.  Further, your current symptoms really don't sound like yeast. But that doesn't necessarily mean they have anything to do with your oral sex exposure 5 months ago, and the negative STD tests you had were reliable.

For all these reasons, you can be very sure you don't have any STD or other infection from the oral sex event.  Still, please see a health care provider for examination to determine the cause of your abnormal discharge and bleeding.  (The mix of STD tests you had suggests you ordered testing on line.  You wasted a lot of money; you didn't need most of them.)  As you were advised on the community forum, your university student health service or a local Planned Parenthood clinic would both be excellent options.  Please do not delay; although the chance you have something serious is low, it isn't zero and you need to be professionally evaluated.
