Yeast Infection or Prostatitis or ???

33 year old male.  Since November of 2010, have been having what I thought were UTI's.  Symptoms:  frequency of urination, frequency of urination at night (once per hour sometimes), ache in left testicular area, 2 nights of waking up sweaty, sore lower back/hips, and my semen volume was reduced, and often had what looked like strands of white material inside it. In the morning usually, I would have a few spots of a clear, mucus like discharge in my underwear.  I went through sexual health screenings, tested negative to diabetes or STDs (chlamydia, ghonnorhea etc) and doctor felt it may be a case of urethritis.  Did Zithromax, did Doxicycline and Cefixme a couple of times but it always seemed to come back again, was put on Cipro for 10 days.  The next time it seemed to come back, Doctor gave me a prostate exam but didnt feel it was overly enlarged/swollen, and put me on 6 weeks of Septra DS.  During this time, I had a small acne outbreak on forehead, but resolved in a week or so.  After all the anti-biotics, I bought myself a fluconozole tablet and took it (because of all the anti-biotics) and my symptoms seemed to resolve.  After a couple of months, it came back again.  NO more discharges or anything, just frequent urination (more so at night, waking up like twice to pee, but daytime urination seems to be normal, about every 4 hours or so).  Generally I go at 7am, 11:30am, 4pm while at work.  At home, I go at like 6pm, 9pm, and then midnight before bed.  Once I've gone to sleep, I usually wake up around 3am and occasionally once before or after that, though it could be because of snoring and I just wake up and realize I have to pee.  Back when this first started last November, I was urinating hourly it seemed.  This whole problem seems to come and go, and once every month or two, I end up with it happening for a few days or week.  Is this typical of prostatitis?   I've changed my diet, lost weight, added in saw palmetto, zinc, pumpkin seeds, plain yoghurt, vitamin c, vitamin e, flax oils, small shots of espresso (for apparent prostate health) but it always seems to come back.  I try to ejaculate 3-4 times per week, semen volume is now back to normal (i think)  Fluconozole seems to do something when I have my flare ups, and things go back to normal for a month or two, though I dont really have any symptoms of a yeast infection.  Wondering if its a yeast in my prostate?  I've not been on any anti-biotics since May and it's August now.  I'm just really frustrated at dealing with this for the better part of the last year.  Any ideas or tips on how to reduce prostatitis?  What else could this be?

get yourself checked for MG,if u havent already,e coli too if u r gay it could be a cause,if not get a prostate fluid examination,that under the microscope will tell if its infected and culture it from there if its possible.if its in the prostate get a course of IM injections,works better than oral neds.mg can show just as a discharge and irritation or not at all ,also azithromycin is coming up resistant so you may have to go 2nd line,,avelox.

Keep us informed if symptoms worsen.  The symptoms I listed were a progression, not all at once symptoms.  The dosage for a Myco induced NGU she gave you is incorrect.  Were talking WEEKS of Doxy or Mino or Clindo.  Myco Gen is a intracellular bacterial infection, that takes weeks to divide and multiply, sometimes up to 28 days to divide.  Any small amount of ABX would cause it to go cystic and wait till the enviroment is more suitable.  It is very hard to culture accordingly.

The symptoms of Mycoplasma dont really fit what I've got before.  I dont have any reactive arthritis, there is no pain with urination, no skin sensations, no IBS symptoms, no neck pain or stiffness, no headaches, eyepain or sensativity to light, or loss of appetite.  My Doctor advised me that she didnt think for a second it would be this due to my sexual history, and that she'd treated us both preventatively a couple months ago in case it was a transmitted sexual bug, which she also felt was unlikely.  

I did go on a long course of Azithromycin back in May to see if it was just a resistant UTI that needed a longer course of antibiotics, I had a 1G dosage, followed by 6 additional days of 250mg twice per day.  This is more than enough to wipe out Myco.  My doctor advised that she'd also done another combination of ABX to rule this one out too earlier, which involved Metronoidazole and Doxicycline.

To clarify she said she didnt feel that my prostate was overly enlarged at the time of the Digital Rectal Exam, but did feel that it was slightly enlarged and it supported my symptoms.

I've been symptom free for 2 days now!!!  But the symptoms were:  frequent urination, weakened stream, stop/start, passing smaller amounts than normal, and not totally emptying my bladder.  erectile difficulties, lumpy semen, reduced semen volume, low back/hip aches, waking up in the middle of the night to urinate, and occasionally constipation that seemed to make the problem worse. Since May, I've not had to go on any anti-biotics, and have managed the 2 bouts I have gotten on my own, by adjusting my diet, implementing more fibre, and exercising.  Saw Palmetto, lots of fluid and more regular sex/masturbation seemed to clear up the issue within 6 days this time....so let's see how long it takes to return again.  

Just before the last bout, I was fine again, but had a runny nose due to a cold going around work, and I took a Claritin to keep it from running all nite, within 12 hours, the urinary symptoms were coming back.  I think this may have been due to the anti-hystimine...

Jessy - Apparently my urine dipstick didnt note lukocytes or nitrates to suggest it was just a UTI, and because whatever it is goes away without anti-biotics, doctor doesnt feel it is recurrent UTI, though it could have happened initially..when all this started.  Anal sex could be the culprit, but she doesnt believe so.  We are using condoms to see if it comes back even with condom use.

Keeping watch to see if it just stays away for some time now..>I need and deserve this break!!!  =)

  if you have NGU issues and having pain issues that come and go ( wax and wane) when using antibiotics, you could have a Mycoplasma Genitalium  infection.  Myco Gen infections will not show up on standard labcorp tests, but you will have worsening symptoms from the unchecked infection.  It needs to be treated w/ long term ABX.  Not a week, not a month, but longer.  

  Your symptoms will be as it progresses, NGU, lower back pain, urination frequency, anxiety, skin sensations, IBS-like symptoms, neck pain/stiffness, joint and muscle pain, headaches, eye pain, sensativity to light, Vit D deficiency, Low B12, , muscle fasticulations, loss of appetite.  What other odd symptoms do you have now?

Are these really the symptoms of MG?  How long does it take to reach these? Is MG found naturally in the body?

I see you report unprotected sex.    Have you looked into non-chlamydia NGU, one cause and another term for what we are talking about here: UTIs.   One is prone when having unprotected sex especially anal sex.   (I do see you wrote "...some bacteria in HIS body"  I don't know if that was a typo or what.  In any case there are articles you can read on NGU and whatnot between monogamous couples.

Well....the doctor said my prostate wasnt overly abnormal, it wasnt boggy but she did say all my symptoms supported it, and based on my urine screen didnt support bacterial prostatits, but possibly the chronic non-bacterial kind.  I am having regular unprotected sex with my partner, as I always have.  It doesnt seem to be any bug travelling between the two of us, but we've started using condoms about 2 months ago, just in case I'm reacting to some bacteria in his body...  

Other reasons for prostatitis:  reduced semen volume, occasional erectile difficulty, having to squeeze the ejaculate out occasionally, lumps in the semen, low sex drive, back/hip aches, rarely, a straining when I defecate, almost like a golf ball is blocking the way....  I've incorporated metamucil, increased my fish intake, and started vitamin e, saw palmetto, zinc, flax, vitamin C, cranberry and a multi vitamin.  

In the past couple of days, I'm certainly seeing a difference...so maybe its improving again...which it has before, and then relapses at some stage.  =)

ON URINATION:

This is probably the wrong forum, but you said you tested negative for STDs.  So that's out of the question.   With the exception of getting up at 3 am to urinate; your urination cycles (7am, 11:30am, 4pm while at work)  sound normal too me (in fact I don't think your urinate enough during the day).   So we know that drinking water in the evening will cause you to get up early to (3am yes) to urinate.  Also, yes urinating once an hour is a bit too much.  But that depends on your fluid intake.

I am about your age, and I urine way more that you... and guess what?  I am STD and prostatitis negative.   I do however drink more fluids than the average person.  2 venti cups of coffee before 2pm and, one naked juice a day, 4 cups of cranberry juice, 4 cups of O.J., and maybe just over half a gallon of water.  

You already had one doctor tell you your prostate was normal, if you want --get a second opinion, after that (that its confirmed normal) you gotta stop thinking its your prostate.

Are you having unprotected sex with your wife/regular partner?
