anal fissure question

I was diagnosed with an anal fissure a couple of months or so ago and am undergoing treatment, which has been slow to work and I find that I have setbacks from time to time. The doctor said there was a second smaller one, almost too small to be called a fissure. They are at the 12 and 6 o'clock positions. My other other symptoms are occasional perianal and genital/penile pain. Can the fissure cause this type of pain? What is the likelihood that an std is causing the fissure given my symptoms and diagnosis?

Anal fissure is different from a STD.
