My guess would be hormonal changes or another infection like yeast or bacterial vaginosis is keeping herpes active for you.
I'd make an appt with your doctor. There are now oral medications you can take, either to just treat outbreaks, or to take every day to help prevent them.
Since you haven't seen a doctor for this in so long, it would be a good idea to talk to one now, to make sure you don't have something else going on, and to talk about medication options should you decide to take them.

also zoviraxZovirax Zovirax topical cream is ineffective for herpes recurrences if that is what you are still using to treat ob's. it's time for some better treatments for you :)
Also there isn't such a thing as a herpes diet no matter what some websites try to tell you. the lysine/arginine theory never held up outside of the test tube. very few folks have dietary triggers for genital herpes. Eat what you want :)

Herpes can change locations. Herpes infects nerve groups, so the virus can change nerve pathways and cause outbreaks in other places.
