can you explain these test results and the NOTES what do they mean, tests taken at 4 weeks 5days and how accurate are they,were they good tests,

You can not ascertain anything from the numeric results except for if they are + or -.  You could've had hsv1 since you were a baby, from the very first person you ever kissed or picked it up last year.  You'll probably never figure it out.

do these numbers say anything to you about how long i could have been positive for hsv-1 and is it true that the antibodies they talk about are the same for both 1 and 2.
what does the 3.4 stand for other than positive
is it true i could have had type 1 for years

As still hurting pointed out - your hsv1 is + and your hsv2 is negative.  About half of folks will seroconvert by the 1 month mark but since you are hsv1+ you should repeat the blood test in another 2 months if it was a high risk encounter.  It's totally up to you if you feel it's worthwhile repeating the test or not.

The notes are just explaining the results and what range the numbers fall in to.

Your test is positive for HSV-1 and negative for HSV-2.

At 4 weeks 5 days these are pretty accurate for HSV-2, the majority would have seroconverted by then, but taking another test at 12 weeks is not a bad idea if you want to rule that out.

Yes these are the good tests.
