scab on penis

i have developed a scab on my penis which looks like a cut from masturbating possibly.  There were no blisters or lesions formed prior to this or prodrome or any pain or burning. I did masturbate roughly the night prior and i think this could be the cause....but usually that heals right away and this is lingering.  The scab comes off after i shower and then reappears in a few hours.  This has been going on for the last 4 days.  Does this sound like a possible herpes infection to you?  Or does this sound like an abrasion of some kind.
Do herpes scabs fall off and crust over again and again like this?  also, is there ever any bleeding involved from the sore?

Dr.H,
I am posting here instead of as a continuation of the thread I submitted a few weeks ago since my situation seems similar and may be informitive and I hope will put my mind at ease a bit since reading this post. I hope this does not breach the protocol of the site. That is not my intention. As you may recall from my prior post I tested positive for HSV1 which seemed likely to be from an older oral infection since I showed no symptoms for almost 12 weeks following our initial contact and several weeks from the last before I tested. I since had oral relations once more with a woman also positive for HSV1 orally (but also had some blisters on her underarm which I noticed after we were done which would have deterred me but probably are not germain to my question anyway)about three weeks after my positive test results were known. Just four days ago (3 wks post incident)following masturbation, I later noticed a superficial cut which looked like I tore the skin. I put neosporin on it and by the next day it was nearly gone and darkened but flat with my skin and by today almost undetectable. From your descriptions "If it started as a bump, then blister, then open sore, herpes is a good bet" also from another post paraphrased ' if you get no symptoms in a week or two you are home free' I see this as different. I also read from the Dermatologist on Medhelp that typically 'the process lasts from 7 to 10 days'. Given that this was three weeks after a positive HSV1 test result and was likely from an older oral exposure which in theory should protect me, healed in 4 days with neosporin and did not pass through those phases that I could see would your opinion of my safety remain the same or do you think this might be genital herpes. It was the only 'lesion' I have seen despite frequent examination. I am very frightened by this as I thought perhaps I had dodged a bullet but now am not so sure and am unwilling to resume sexual contact with my wife. Thank you again for all you are doing.

I also have a smilar question. I have never had unprotected sex and oral a few times more than 6 months back. Just recently I realized a small scab on my penis shaft about half way down. I have never had this before, it never started as a bump but the scab fell off easily and now there is a small spot of a pinkish skin. I do admit I do masturbate, can it just be a cut?

Actually i got the culture done the first day i noticed anything on 6/1 and recieved the results on 6/6....do you think thats enough time for the test to be accurate?  I've been asking your opinion as somewhat of a second opnion since you have such a vast knowledge of this type of std. the original dermatologist told me she thought it was just a scratch...i then visited another doctor on monday who thought it could be possibly syphilis, a scratch, but not herpes.  He stated that the absence of pain in any form would rule out a herpes infrection. Ive just been incredibly neurotic about this since the appearance of this thing on my penis.
Anyway, do you think that being tested the day of noticing the mark would be a good precursor to an accurate test?  Ive heard the culture tests can be unreliable.  I would also like to thank you for this informative and helpful service that you provide....the wealth of knowledge on this website has been of great educational value to me.

That's good.  However, it doesn't rule out herpes, because tests for the virus always become negative as a lesion heals.  To know for sure, you need a type-specific blood test, such as HerpeSelect.

For what it's worth, I doubt your test was a culture, which takes several days to know it is negative.  Most likely it was either PCR (the best) or an antigen detection test, such as a fluorescent antibody (FA) test.

i have one more question if you dont mind.  The scab has fallen off and healed now after 4 days...have you ever heard of herpes doing something like this? by the way, i went and saw a dermatologist yesterday who cultured the scab...she thinks its a cut, but to be on the safe side took the culture.

thanks

Genital herpes is by far the most common cause of genital ulceration, even in people who believe other explanations (including injury) are more likely.  I remain skeptical about masturbation as a cause of such lesions; it seems more likely that it exacerbated something else that otherwise would have remained subtle.

If you're so convinced your lesion was due to injury, presumably you would not have asked my opinion.  I have given it.  Do as you wish.

this is the first time this has appeared here...there was no bump or anything like it before it appeared...the skin was just irritated from masturbating and the next day a small scab showed up.  Also, its on the very bottom of the shaft near the base...could this be a spot that herpes shows up in?  Ive been reading that its usually on the head or where the head meets the shaft?

On re-reading your question, you seem to be saying a similar lesion appears from time to time in the same spot.  That makes herpes sound like a pretty good bet.  Masturbation sufficiently vigorously to actually rub the skin raw or cause a cut sounds pretty unlikely.  You should see a herpes-knowegeable health care provider.

A scab is a scab; that is, one doesn't look much different from any other.  What a lesion looked like before it developed a scab is a much better clue to the cause. If it started as a bump, then blister, then open sore, herpes is a good bet.  Otherwise I cannot help you distinguish the various possibilities.  If you're unsure, you need to see a health care provider.
