std concern

Doctor i asked you a question a couple of weeks ago about white spots i was seeing after squeezing my penis, i was having anxiety over a encounter i had 5 weeks prior with a married women i am married also, i took your advice and stopped squeezing and have seen nothing sense, but three days ago i noticed a little burning after i urinated so today i went to my gp and they took a urine sample and they saw trace amounts of wbc's they said it was a uti, i had a urine test done two weeks ago and they said everything was clear, so can you have chlamydia experience burning and not have a discharge of any kind, also the women promised me she was not infected with anythin because she has had problems with her back and she gets her blood and everything checked every month.

A suggestion you may want to discuss with your doctor.  Ciprofloxacin, while a good therapy for many UTIs is not very good therapy for nongonococcal urethritis. On the other hand, a related drug, levofloxacin is, as is doxycycline.  If you and your doctor decide to go the treatment  “route” as appears to be the case, treatment of your partner may be a good idea as well, not because I have any great suspicion that an STD is involved but because it is the most conservative approach, given the decision to peruse treatment.  EWH

Dr. Handsfield and I share the forum.  You got me this time.  FYI, the reason we share the forum is because we have worked together for nearly 30 years and while our verbiage styles vary, we have never disagreed on management strategies or advice to clients.

I reviewed your prior interaction with Dr. Handsfield and agree with everything my colleague has said.  Perhaps I missed it but while I realize you had a urinalysis, did you get specifically tested for gonorrhea and chlamydia or not and did you receive any treatment, either at that time or for your recent UTI diagnosis?  I do not see that either happened.  The reason I ask is that a urinalysis is a non-specific test while the other tests I mentioned are more specific.

Similarly, burning on urination is also a non-specific symptoms and while it may result from STDs, it may also be detected when people are hyper-vigilant and concentrating on whether they have symptoms or not.  Most men have sensations which accompany urination which they tend to ignore unless they are anxious or concerned. When anxious or concerned however, these otherwise normal sensation can become more evident and worrisome.

The fact that you had an earlier urinalysis which was clear makes the probability that your current burning is due to something acquired in your earlier encounter lower, particularly if the urinalysis was performed on a specimen collected at the beginning of urination and after at least an hour had passed since previous urination.  Both of these increase the reliability of urinalysis for detecting STD/

I agree that the partner you mentioned was low risk and that the probability of your getting an STD from the encounter you described is slight.

Hope this helps. EWH
