5 years is a long time not to have any kind of issues with an STD.
It could be a new STD infection, it could be NGU which u can get from mouth bacteria when oral sex is performed on you. it could be prostate issues.
Best advice is to see a Dr at this point and get a proper diagnosis for what it is. If you get tested for STD's if any come back positive I would recommend retesting before taking medication just to be sure it was positive and not a false positive.
