Do I need a shrink or a urologist?

It's been over a decade since I've had a sex partner or have had any sexual relations with anyone. So a few years back I started pushing my penis inside myself for gratification. Problem is I don't know how normal this conversation is going to be. I was purchasing dildo's to shove my penis further into myself and at some occasions I felt a ping of sharp pain almost like the skin or muscle inside me just detached itself. I would slow down and work it in slower till it went full hilt. About 2 years ago I started using double sided dildo's to make it a hands free experiment. This year for a resolution I decided to stop doing this to myself because I was beginning to wet the bed and didn't even know I was till after I had woken up. I think I need to see a urologist because I tried pumping my penis out of my body and it came out as a big bulge of muscle instead. How would one even approach a urologist let alone your PCP?  Today when I use the bathroom I've grown accustomed to the burn and have had 4 UTI's last year alone. Now one thing I want to say is I swing both ways and it's not a gender psych problem.

It sounds like a physical exam is in order to be sure you have not separated any muscles that are important for bladder support or other reasons.

Get an appointment with a urologist as soon as possible to fix this and quit doing this.  I am surprised a doctor seeing this for the antibiotics needed for UTI has not directed you accordingly.

You need to be frank with the urologist and tell him everything!! Doctors have seen and heard it all.
