Foreskin problems

I have several issues related to my foreskin. I'm 17 and I've had no sexual contact with anyone.

1. My foreskin is hard to retract, I went to a urologist and he said I had a higher frenular attachment. He told me the only way to fix it was circumcision. I can retract my foreskin when flaccid pretty easily, but if I'm even semi-erect it's pretty difficult. It's impossible to do when fully erect, literally impossible. I've tried pulling back the foreskin while flaccid and making myself erect (by thinking about sexual thoughts) to see what would happen and I found that when I got fully erect, I felt quite a bit of pain. I originally thought it was phimosis but I was told that since I did have the ability to retract my foreskin while flaccid, and there appeared to be no scarring or reason as to why I shouldn't be able to retract my foreskin other than tightness when erect, that it was not the case.

2. My penis is very VERY sensitive. When the urologist pulled my foreskin back to examine me and he had gloves on, he touched my penis head to examine it and I had to grab the table to keep myself upright. Afterwards my penis hurt for at least a good hour to hour and a half afterwards. I can't touch the head without at least some pain and it makes it nigh impossible to try and clean. The most I can do is run hot water and soap over it. I can't even rub it at all.

3. There are these little tiny, what look to be salt crystals at the bottom of my penis head. I'm told this may be smegma but I've tried to rub it off (rather painfully I might add) and it doesn't seem to want to come off.

4. For some reason, my foreskin goes at a 45 degree angle to the right. When I get fully erect, I still have a stub of foreskin at the end of my penis.

I'm really confused and I've gotten to my doctor several times about this but nothing seems to be working and I feel like my doctor is missing the mark on these issues. I'd appreciate some help.

You did mention that you have pain in the head of your penis and it has been difficult to touch and wash in your first post.
Sorry if I misinterpreted what you wrote.  But do hope things settle down for you.

But in any event do not use normal soaps to wash down below, use latex free condoms and as a home help remedy should you experience problems with itching or soreness, applying natural set yogurt to your genitals, this does help.

Thank you for your response but I don't think that it is an infection. I didn't mention it in the question but this isn't anything recent, it's been afflicting me for years and so I don't think that's where the sensitivity is coming from. I've examined my penis for quite a long time to try and find some problem, but it just seems to be a normal penis. There is no swelling, no pus, no abnormal discharges, nothing that seems to indicate any kind of infection. I thought that the pearly pastules might be a clue but I found out that they are normal. As for seeing another urologist, I'm not sure what I can say to see another as he told me my options and nothing new has happened since the examination so I'm not sure if my insurance would allow me to see another urologist. We've recently switched insurance and I'm 99% sure that my new general physician thinks I'm a hypochondriac so I'm not even sure if he'd be willing to write me a referral for the same issue again.

Personally I feel that you require a second opinion.  With your penis being very painful and tender this is could be Balanitis caused through either a fungal or bacterial infection.    As your foreskin is also affected then this  is called balanoposthitis.

A swab test would confirm whether you have an infection and if it was from  fungal or bacterial infection.  Appropriate medication would be prescribed.

Wash your hands before using the toilet and again after using the loo so as to avoid spreading bacterial.

When you use condoms use only the anti allergy ones and those that are latex free.

Thank you for your responses. I looked up pearly penile papules and it is definitely what I have, no question about it so thank you so much for helping me with that. Though I still have some unanswered questions mainly the tightness of the foreskin and the extreme almost painful sensitivity. If a doctor's simple examination can leave my penis in pain for 1-2 hours after the examination, how could I possibly expect to function without pain during sexual intercourse? especially if my sexual partner has a hymen. Thank you all so much for your responses again but I would really like the sensitivity and tightness issue cleared up please.

The bumps are probably pearly penile papules or one other, similar benign condition that I can't think of the name of at the moment.  They are not terribly uncommon and won't hurt you and are not contagious.  

Since the doctor you saw wasn't smart enough to tell you what they were, I would see another urologist for a second opinion on your foreskin issue to see if there is surgery that would loosen the foreskin instead of remove it.

As long is it is not making your masterbation painful, you should be able to perform normally during sex. I have had sex with 2 seperate men with very tight foreskin, never circumsized and they performed just as well, if not better than others.

If it was smegma that you have, this would wash off.

Do not use any soap to wash your penis.  Soap dries the delicate skin.  You can get soap that is not perfumed and one that has a pH level that suits the skin.  Baby washes should be fine.  Don't use any shower gels.

With pain at the tip of your penis and the white spots, you may have a fungal infection.  The doctor would be able to prescribe an antifungal cream for you.  You can also purchase this from the pharmacy.  These are antifungal creams specifically for the genital area, so make sure you get the right one.  You can also purchase from the pharmacy a one dose single oral capsule called Fluconazole.  Although these should help if you have a fungal problem, it will not help the tight foreskin.

As a home remedy after you have washed and gently dried your penis, you can apply lots of natural set yogurt.  This is best done before bed time.

Don't wear tight clothing and only wear cotton underwear.  Using a non-biological washing detergent may help.  

Cut down on sugary foods and drinks and drink more plain water.

If you are still having the pain and the white spots on the penis, make an appointment with a different doctor.  A swab would indicate if you have a fungal or bacterial problem.

Use KY jelly or similar to lubricate and help retract the foreskin.  Unfortunately, an operation may be the only option open for you to correct this problem.
