Yes, when the hymen is broken it will come out immediately mixed with blood. You will wipe yourself and possible have slight discharge for the penis rubbing against the uterus wall, causing the temporary bleeding and discharge. Your second experience you will also see just a little discoloration, but this is all normal for first timer.
Yes, it is possible for the hymen to come out. I have experienced this myself. If it is tore in a certain uneven way, it can hang out of the vaginal opening almost like a third labia.(Also called labial hymen) Very rare, but does happen to some of us. I was told by the gyno that it can be cut out and it is a pretty simple procedure. Not a lot of info on it on the internet. It is harmless though.

i wouldnt think so... its just stretched across the opening and rips, so it probably just retracts to the side...
