Put a spermicidal condom on the wrong way. Help.

Hey guys, just wanted some light shed on one of my concerns.

My girlfriend and i had intercourse 2 times in one night. I used OKAMOTO nonoxynol 9 spermicidal condoms both times. I ejaculated in the condom while outside of her both times.
After the first time, i removed the condom, and urinated. After a few minutes we then decided to have intercourse again, so we proceeded to making out for awhile. After i was aroused, i went to the bathroom to urinate again and wipe any remnants of precum or semen from my penis before putting on a new condom for our second time. I then noticed that the condom would not go on properly because i had it on backwards. Therefore i inverted it, and put it on the RIGHT way. Knowing that there could have been precum left on the tip of my penis, i wiped the tip of the condom which touched the hole of my penis (when i initially put the condom on the wrong way). To be sure, i again dabbed it with wet toilet paper. After this, i then proceeded in entering my girlfriend.

I just wanted to know several things:
1. What is the probability that semen from my first ejaculation was left over in my urethra after i urinated twice (and wiped) before having intercourse for a second time?
2. What are the chances that the mixture of precum, urine, and maybe semen ended up on the outside tip of the condom (when i  put it on wrong the first time) even after me wiping the part of the spermicidal condom which was exposed to the tip of my penis? And is there a strong probability that this "substance" could find its way into my girlfriend and cause a pregnancy even after all of my precautions?
3. Or am i just being paranoid?

Would appreciate an answer soon. Thanks.
