If this happens both while awake and asleep it seems it could be a pulmonary issue. If excess weight is an issue, the shifting of weight when lying down can restrict breathing, and even cause sleep apnea.  In any case, if there is any doubt, it seems prudent to be sure there are no lung problems going on. Respiratory function tests can measure lung volumes and the ability of the lung tissues to process air.

What if you were to lay on the bed in the same position without a pillow? Still a problem? And are you talking about laying on your back or side? And is weight an issue? It would seem that one could have the symptoms while lying on the back on the floor but not in the bed. The floor does not give like the bed would to the weight of the lower body, so on the floor the upper body could be lower than the lower body, thus increasing pressure in the chest. Beds are designed to contour to varying degress to the body to keep the body more level. I'm just doing a whole lot of thinking out loud.

I have a friend who can't lay on their left side due to valve problems.

I'm not asking these questions because I have answers, but more for you to more clearly define your issues. If you do decide to consult a doctor, be very exact with your positions and symptoms. Then the doctor will be better able to recognize what possibilities your very specific symptoms indicate.

If it were me, to be safe I would want to be seen by a doctor for a respiratory and cardiac workup. The symptoms have to be coming from somewhere.
