Falling asleep suddenly for seconds at a time

I fall asleep frequently for seconds at a time throughout the day.  My wife says that my body twitches during these episodes.  I fall asleep while people are talking to me.  I dontbknowthat I have fallen asleep, my wife tells me I do.  I have one carotid artery that is 100% blocked. I am a renal dialysis patient. And I have heart disease

I was reading about sodium today, and the symptoms of fatigue and twitching came up as symptoms of low sodium.  

Because you are on renal dialysis, you should discuss your symptoms with your doctor to find the cause so that you can get the appropriate treatment.

There are a few reasons why you may be dropping off to sleep.

You may not have adequate sleep at night, from some infections, lack of some vitamins (B12) and iron, boredom, sugar levels, or it may be related to your heart disease.

Make an appointment to see your doctor to run some tests to see if there is an underlying cause that you may require treating.

It appears you are having epileptic episodes, of unknown etiology. I would describe your problem as "falling asleep". You require an in-person evaluation by a neurologist, ideally one experienced in epilepsy. There are medications that control the problem. It could be due to carotid bklockage, howeverthe "twitching" suggests this is not due to simple oxygen deficiency secondary to blood flow. It is above my pay grade to make a differential diagnosis. Arrange an appointment as soon as possible. There is evidence that there is cumulative brain cell loss associated with untreated siezures of this natrure.
