Delusional, crazy talk after stroke

My uncle, 45 years old, had a stroke on Feb 28 while in the hospital for minor shoulder surgery.  He had some numbness on his left side, but then he was fine. His left foot was tingling and then troubling him with severe pain, they diagnosed it as Gout.  He came home from the hospital and in the last two weeks has been acting like a crazy: ranting, raving, making up stories, cursing, no memory, severe mood swings, paranoia, severe ansomnia and incontinence.  He was admitted into the hospital while on vacation and they did a series of tests.  They did MRIs, CTs, spinal tap, took him off his medication, gave him a psychiatric evaluation, he failed the dimensia test.  At first they thought he was bipolar, but they have no idea what is wrong.  Any ideas?
