Jaw pain, crunching caused by motor tic, where should I turn?

I am a 30yo man experiencing intense pain in my jaw.
I've had mild tics since adolecence, usually only 1 at a time.

For the past few years one has stuck around, a clenching, thrusting of my left jaw joint.
I can't stop, and i am afraid I have damaged my jaw, it crunches when I move it, and it hurts
like hell in the morning, sometimes waking me up at night.

I am already on various anti-anxiety meds, but I can't seem to shake the tic
and now I'm stuck with this pain.

What should I do? Address the tic first, and then the jaw damage?
My doctor/psychiatrist/psychologist never seemed too concerned about my tic

Have any dentists dealt with this before?

The tic's might stop, if you start wearing a nightguard. I can not belive know one as told you to go to the dentist, or x-ray on your jaw. But if I know better being a man you to do not want to say anything. You could try a nightguard @ the store 1st, but I have had tmj since the age of 24. I hate to go to the dentist because these found my tmj, I had a fake toothache, after alot of dds's one finally ask if I had headaches and I did.  This was in 1982. Good luck let me know what you fine out.
