late implant infection

I had a dental implant placed 14 yrs. ago, with no complications.  I now have an infection at the distal end of the implant(sinus end).  The infection is localized there and my dental surgeon tells me the infection did not start at the gumline since my bone along the implant is fine and there is no imflamation at the gumline.
The xray of the base of my sinus looks fine and my dental surgeon has no idea how the infection got there.
He is going to clean the area out and do a bone graft in three days. Has anyone heard of this before. I'm 45 and in excellent physical health.
